Job summary

We are excited to be able to offer a 1 year fixed term contract for our Hospital @ Home Team Lead position.

This is an exciting opportunity to help develop our newly created Hospital @ Home Team working alongside local acute Trust's to provide early and safe discharge from hospital and hospital admission avoidance for our adult population. We are a 7 day a week service and the Team Lead is a vital part of this clinical work.

Main duties of the job

  • The post holder will work as an autonomous expert practitioner with responsibility for leading a team of nurses, providing highly skilled & developed quality nursing care for adults in their own homes and other community settings.
  • To develop, implement and evaluate a seamless Hospital@Home (H@H) Service, ensuring that patients receive the highest standard of clinical care.
  • To work as an autonomous practitioner with responsibility for facilitating early discharge, reduce length of stay and prevent hospital admissions.
  • To provide continuing responsibility for the assessment, development, implementation and evaluation of nursing care for patients.
  • To work in partnership with members of the multi -disciplinary team to maintain high quality care & promote practice that is patient centred.
  • To continue to develop and maintain close links between Community and Hospital based services.
  • .To be responsible for providing support to junior team members through developing their skills & completing staff appraisals. They will motivate staff to provide high standards of care by acting as a role model.
  • To undertake the organisation and support of student placements & mentorship through their practice placements.
  • To undertake and implement research and audit activities to ensure evidence-based practice and best practice guidelines are delivered within the service.

Providing local leadership, consultancy and advice for nurses, other professionals, patients and their carers.

Developing close working partnerships with Community care providers; local and regional specialists; other health professionals and voluntary services involved in the patients care

Lead and co-ordinate competency based training and education within the service

Putting the patient at the centre, reinforcing the concept of a streamline patient journey, improving total patient experience. The role will involve cross-boundary and inter-agency working.

Working with our clinical leads and integrated Multidisciplinary teams across organisations to develop the model for NHS@Home virtual ward universal personalised care within the Virtual Wards.

Incorporating digital capabilities with the efficient use of technology to support clinical decision making
