An exciting opportunity has arisen for an individual to join our team as a Telecare Assessor on aone-year fixed term contract. The successful candidate will develop an enhanced skill set that is transferable throughout the NHS and other public sector roles within a Health and Social Care setting. Full training and regular supervision will be provided. This position is being offered on a fixed term / temporary basis, but there may be scope for this role to become permanent.

This is a very rewarding role, the post holder will feel a great sense of job satisfaction as our equipment supports vulnerable adults to remain living safely and independently in their own home, enabling individuals to have more choice and control of their lives.

The role requires travel throughout the County, visiting vulnerable adults with varying disabilities and needs. Therefore, access to a vehicle will be essential for this role.

Main duties of the job

The successful candidate will be responsible for assessing individual needs by visiting individuals in their own homes, hospital, or other care environments andwill be expected to effectively manage their own diverse caseload.The post holder will prescribe Telecare Technology solutions and look at situations holistically and refer onto other services, where needed. This will offer a great insight into the tech world that we are entering and see how technical solutions can help support those with Health and Social Care needs.

The Specialist Telecare Assessor is responsible for the assessment, review and re-assessment of assistive technology packages, including basic installation of Telecare devices / sensors.

The post holder will use their practical and technical skills to problem solve, evaluate and alter Telecare care plans within set eligibility criteria. They will also install and fault find on basic Telecare equipment over the telephone and within a person's home.

The ideal candidate will be a team player, will be prepared to work hard, display a positive & caring attitude, understand a range of health conditions, and be able to effectively communicate with both service users, their family and other health & social care professionals.
