Job summary

We are looking for an enthusiastic experienced Doctor to join on a part time basis our community hospital in the Vale, (Dursley Gloucestershire).

The Community Hospital Doctor will work autonomously and in partnership with Advanced Clinical Practitioners, to provide medical services to an inpatient community hospital population, including inpatient beds to support the countywide stroke rehabilitation pathway.

The Doctor will act in a medical role, to assess, plan and manage medical risk, to support service users and the wider MDT in a medical capacity.

Where asked to do so, the post holder will work in partnership with ACPs or trainee ACPs, to identify and manage the needs of the ward and patients, to manage medical risk and complexity, to provide training and direct clinical supervision.

Full induction including time with the stroke team will be provided.

The holder will practice in accordance with the GMC's code of professional conduct and within the local policies and pathways of GHCNHST.

The role will enhance multi-professional working with nursing, medical staff and allied health professionals through role modelling, managing medical risk and complexity, and offering a holistic set of skills to facilitate ongoing biopsychosocial assessment of service users.

The post holder will support activities to monitor and improve standards through supervision, evidence-based practice, clinical audit and research.

Main duties of the job

  • Practice at a high level in assessing and diagnosing patients with a range of clinical presentations and work towards developing a core understanding of Frailty and Stroke specific presentations.
  • Demonstrate a high level of knowledge in relation to pattern of disease / disorder, markers of condition progression and range of treatment available at each stage of disease or condition.
  • Exercise professional judgement to manage risk appropriately, especially where there may be complex and unpredictable events and supporting teams to do likewise to ensure safety of individuals, families, and carers.
  • Work collaboratively with MDT colleagues to ensure the best active and ongoing care of service users, including effective discharge and onward care.
  • Ensure timely and effective Advanced Care Planning/ReSPECT discussions within the MDT and with service users.
  • Work with ward colleagues to anticipate and manage the needs of the patient who may be entering a dying phase, including parallel planning where indicated, effective palliative support and care in the last days of life.
  • Establish, maintain, and effectively manage barriers to skilled and effective communication with varying parties
  • Participate in clinical supervision to support colleagues in reflection on practice and improve knowledge and improve the quality of care to service users.
