Job summary

Jubilee ward is a multidisciplinary led service that provides sub-acute general medical care, following a period in an Acute Hospital setting.

The ward prides itself on the use of the enablement model ensuring patients and their carer's are at the heart of everything we do.

Main duties of the job

  • Deliver a safe and effective patient centred care to our service users, their families and carers
  • Ensure all clinical care requirements of patients are communicated effectively at ward handover to allow seamless provision of care from shift to shift.
  • Responsible for the safe use, inspection, maintenance and security of all goods and equipment, ensuring appropriate stock levels are maintained at all times.
  • Responsible for the checking and safe storage of patients' property and valuables in line with the Trust's policy.
  • Implement and conform to Trust and Departmental Infection Control policies and procedures. To use non-medical prescribing or Patient Group Directives (PGDs) where applicable and to ensure that all duties in relation to medicines optimisation/management meet required professional and Trust standards.
  • Responsible for ensuring patients records are kept up to date, accurate and in line with the Trust's Record Keeping Policy and Medicines Management Policy.
  • Responsible for assessing, planning, implementing and evaluating care for a specific group of patients. Undertaking comprehensive health care needs assessment of patients and reassigning as appropriate.
  • Undertake joint assessments and responsibility for initiating, co-ordinating and implementing discharge planning involving the Multi-disciplinary, Primary care, Mental Health and Social care Teams where indicated; making onward appropriate referrals.
  • Participate in Doctor's rounds.
