Senior Community Physiotherapist - Band 6
Gloucestershire Health and Care NHS Foundation Trust

Job summary
An exciting opportunity has emerged for an individual to join our Integrated Community Team (ICT) working in Cheltenham. The ICTs consist of Community Nursing, Physiotherapy, Occupational Therapy and Reablement who work closely with our Adult Social Care colleagues.
We work in partnership with GPs, community hospitals, mental health services and voluntary organisations to enable people to live as independently as possible at home. The ethos of the team is a reablement, person-led approach to recovery and self-management, to ensure we offer care close to home, thus avoiding unnecessary admissions to bed-based services.
This is a multifaceted role with a varied caseload incorporating both triage and urgent response of patients referred via the referral centre, and patients from the community waiting list. The post holder will also be responsible for the delegation of work to other members of the physiotherapy team within the ICT, as well as holding a caseload. This is an ideal opportunity for a Band 5 therapist wanting development or a Band 6 looking for a new challenge.
We can offer you the opportunity to work in a very supportive integrated team, with access to training, regular supervision, annual appraisal and the autonomy to be a lead professional in our new service delivery. We welcome interested colleagues to phone for an informal chat. We can also accommodate shadowing if you would like to visit and spend time with our extremely friendly and supportive team.
Main duties of the job
To work as a Band 6 Physiotherapist participating as a member of the community team and independently managing a specialised clinical caseload. At times you might also be required to manage and triage work within the Cheltenham Referral Centre.
The post-holder will assess, formulate, evaluate and monitor specialised rehabilitation care packages which will involve complex decision making within the community, including Home First services, bed based reablement and referral centre. The role can require urgent responses to priority cases, and will require negotiation and co-ordination of a number of multidisciplinary team members to deliver specialised rehabilitation care programmes.
About us
Gloucestershire Health & Care Services deliver community health & social care services for adults & older people via a model of locality-based cluster teams. Each of the clusters consist of Nursing, Therapy and Reablement Teams working closely alongside social work professionals to provide support to people within their own homes and within bed based reablement units. We work closely with GPs, local communities, acute and community hospital(s) and other partners to enable people to live as independently and for as long as possible within their own home.
We are a clinically led organisation where individuals are valued for their unique professional contribution to ensuring best patient care. Continuing Professional Development is a key priority and we actively promote and develop the broadening roles that Physiotherapists and Allied Health Professionals are able to contribute. We have an extensive range of Advanced Practitioners, Clinical Specialists, Professional Leads and Non-Medical Prescribers across a wide range of specialisms that actively support clinical colleagues within our organisation. Our well established, structured Clinical Supervision and In-Service Training programmes contribute to the learning culture which develops and challenges thinking and practice.
