Job summary

We have an exciting opportunity for an Occupational Therapist to join our team in Cheltenham, This is a key role working as part of our Integrated Community Team, alongside our partners in Adult social care, and primary care, providing rapid assessment of patients in their own homes, to prevent hospital admission and facilitate hospital discharges.

The caseload is wide and varied, and you will require robust clinical reasoning and problem-solving abilities, working in a fast-paced environment. We have a very supportive and cohesive MDT working together to support our patients to live as independently as possible in their own environment.

Recently we have rolled out a successful service development utilising the skills of therapists in the assessment of the deteriorating patient, carrying out NEWS 2 assessments with all new patients to the service. This has had a positive impact on our patient care, expanding our AHP skill set in line with the requirements of our ever-changing NHS services, placing AHPS at the forefront of such developments in Gloucestershire.

Main duties of the job

As an Occupational Therapist you would be a key part of our team, providing assessment, goal planning and reviews, working alongside Physiotherapists, Nurses, Reablement workers and rehabilitation assistants to enable our patients to gain their maximum level of independence in their own home. The caseload will be varied working with the MDT in the referral centre to carry out rapid assessments to prevent hospital admission and facilitate rapid discharges, including falls response work within agreed countywide timescales.
