Job summary
- The Inpatient Wards at our Community Hospitals are at the heart of what we do. The inpatient Wards offer a round the clock - 7 days a week service to provide a high standard of care and support for patients where care can no longer be delivered in the home due to the nature of their condition.
- To provide and maintain standards of care as required by the NMC Code of Professional Conduct and local and Trust Policy.
Main duties of the job
Make appropriate referrals to other professionals and agencies e.g. Social Care, Community Nursing, Intermediate Care, Voluntary Organisations.
Facilitate and encourage patients, carers and relatives to take an active role in care given.
Make appropriate use of knowledge and clinical ability to recognise changes in patient's physical/mental condition inform others of identified problems/needs and facilitate solutions.
Act independently and be responsible for own actions.
Use knowledge and skills to teach and provide clinical care that is evidence based.
To ensure all clinical care requirements of patients are communicated effectively at ward handover to allow seamless provision of care from shift to shift.
- Participate in doctors' rounds and multidisciplinary team meetings.
- Act as or contribute to the role of Speciality Link Nurse to other areas, ensuring feedback to ward team enabling review of practice.
- Undertake comprehensive health care needs assessment of patients, reassessing as appropriate.
- Implement, maintain and monitor agreed standards of care and participate in clinical and nursing audits.
- Accountable for the models of nursing care and ensure nursing care is delivered in a professional manner compatible with current evidence/research.
- Responsible for both providing and supervising the direct nursing care for all the community hospital patients on in-patient wards.
