Job summary
As part of community mental health transformation we are seeking to develop the community rehabilitation pathway across Gloucestershire Health and Care NHS Foundation Trust with a particular focus on individuals with treatment resistant psychosis and frequent admissions or long stays in inpatient units locally, and out of county, as well as individuals with 24 hour support whose placements are at risk of breaking down.
This newly established team will seek to build links between internal services, VCSE (voluntary, community or social enterprise) organisations and existing/new care providers to ensure a multi-agency, collaborative and recovery-oriented approach to complex rehabilitation. This will include creating a shared ethos and goals and a sense of hope and optimism to deliver individualised and person-centred care to promote independence and integration within the local community.
Main duties of the job
As Team Administrator for Rehabilitation Pathway Service the main duties would be:
- Use Trust systems to input and look up electronic data as required and have a good understanding of RIO pathways and admin responsibilities in accordance with Trust policy
- Ordering, storing and managing of stationery using the Trust's electronic procurement system in line with the Trust policies as and when required
- Typically, be the first point of contact for all patients, staff and external stakeholders face-to-face and via phone and email dealing with queries and taking/relaying messages. Have a helpful and professional approach and show exceptional patient care at all times
- To use Microsoft Office packages to produce reports, spreadsheets, presentations, and emails to a high-quality standard
- To maintain diaries for the team
- Photocopying, scanning, printing when required
- Arrange staff and patient meetings and appointments as necessary
- Actively support team meetings by co-ordinating, collating and ensuring accurate and timely distribution of papers, as needed. Ensure that Minutes are accurate, well written and presented, and effectively distributed in a timely manner
