Job summary

The Intensive Health Outreach Team (IHOT) was established in 2008 commissioned to provide support to people over the age of 18 who have a learning disability.

The Band 5 Registered Learning Disability Nurse will :

  • Ensure access to mainstream health services, to allow for equitable health outcomes comparable to the general population.
  • Strive to strengthen the NHS contributions to prevention of, and tackling health inequalities within acute, primary, secondary and community care, working in partnership with services to make reasonable adjustments; a legal requirement to make sure health services are accessible to all disabled people.
  • Ensure equal opportunities in health care for people living with a learning disability. The team operates a county wide service within Gloucestershire.

Main duties of the job

  • Develop individually designed packages of care and treatment, in association with other care professionals on a multi-disciplinary basis and under the supervision of the team leader
  • To maintain and develop specialist knowledge about conditions and factors commonly associated with people who have LD, their causes, impact and appropriate interventions.
  • Report, document and evaluate all clinical practice according to local policy (i.e. risk assessment and consent) and Nursing and Midwifery Council guidelines. When appropriate maintain records .
  • Provide current, researched education, support and training to clients, carers and staff to promote, inform and teach healthy lifestyles.
  • Supervise junior staff as required and input into training new staff and students.
  • To be able to communicate to people with a wide range of abilities and difficulties
  • Reflect and review own practice through actively participating in clinical supervision,

Continuous professional development and appraisal.

  • Maintain and update own professional knowledge skills to provide an evidence based
  • Practice through continued professional development including specialist courses and maintaining a professional portfolio.
  • To be competent in IT skills in both inputting and gathering data.
  • Manage the team in the absence of the team leader
