Job summary
The Complex Care at Home Service (CC@H) is looking to recruit an experienced physiotherapist to become part of the team. GHCNHSFT CC@H service provides proactive, preventative case management to a mainly frail older cohort of patients living in Gloucester, Cheltenham and the Forest of Dean. This position is based in Cheltenham, but the role requires the person to be able to support their colleagues in the Forest of Dean as required.
The team consists of matrons who are nurses and allied health professionals, therapists, wellbeing coordinators, care navigators from adult social care, frailty housing officer and clinical support and advice from consultant geriatricians.
Main duties of the job
You will be visiting people in their own homes and supporting them to live well and improve their resilience, with a focus on what matters to them and their concerns.
Our team love coming to work in a role that allows them to use a health coaching approach to support a person achieve their goals. We always try to use Health Coaching and Motivational Interviewing skills and utilise outcome measures that evidence their effectiveness in improving a person's health and wellbeing.
The CC@H service is a truly multi-disciplinary team who support people living with complex health conditions and frailty to remain as independent as possible in their choice of residence. The role is rewarding and challenging but never boring. You will be supported by an amazing team of clinicians, health and wellbeing coordinators and social care colleagues.
The service is committed to your career progression and we encourage and support Masters level modules and opportunities to develop within the service. Career progression can include clinical pathways such as Advanced Clinical Practitioner and Community Matron roles, all within the service.
