As we transform our inpatient teams we have embraced a revised skill mix to create a blended medical model to include ACPs, Doctors and a Clinical Pharmacist. Stroud and Vale Hospital (in Dursley) collectively have 58 beds mainly focusing on providing sub-acute recovery, rehabilitation, frailty assessment and management and palliative / end of life care, along with specialist stroke rehabilitation at the Vale hospital.
It's the perfect time to join our journey as we shift our Community Hospitals up a gear and embrace extended roles; diversifying the skill mix, thus creating a tapestry medical cover approach and improving our MDT offer.
The successful candidate will be a fully qualified ACP, accountable for the leadership, development, improvement, and management of patients within the clinical areas ensuring the highest standards of patient care is delivered in the most efficient and effective way across a 24 hour period, 7 days a week. You will have consolidated prescribing practice and experience in the treatment of long term conditions, frailty and palliation. Using critical thinking, you will make high level skills decisions, informed by advanced history taking, diagnostics, assessment and treatment skills. Support will be provided from the Clinical Director for community hospitals and urgent care, supporting doctors and current ACP's within the trust.
Main duties of the job
You will need to be a dynamic, established ACP having completed an Advanced Clinical Practice Masters programme awarded by a University with Health Education England Centre for Advanced Practice accreditation and holding a HEE digital badge
Or an established ACP with evidence of an Advanced Clinical Practice Masters programme willing to commit to progressing through the Health Education England Centre for Advanced Practice e-Portfolio (supported) route to gain HEE AP accreditation and be awarded a HEE digital badge ePortfolio (supported) Route - Advanced Practice (hee.nhs.uk)
- You will provide clinical support for the inpatient wards in Stroud and Vale community hospitals
- You will be responsible and accountable for the leadership, development, improvement, and management of services within own defined clinical areas to ensure the highest standards of patient care is delivered in the most efficient and effective way across a 24 hour period, 7 days a week
- Act as an autonomous Practitioner in accordance with the 2017 HEE AP Framework providing high quality, individualised clinical management for patients using advanced clinical assessment and diagnostic reasoning skills to determine and initiate changes to treatment as necessary.

QUALIFICATIONS
Essential
- Registered practitioner with a Statutory Regulated professional body (e.g. NMC / HCPC) with significant post-registration experience relevant to role.
- Masters in Advanced Practice or evidence to support Professional clinical knowledge that covers all 4 pillars (as above) of the Advanced Clinical Practice capabilities (essential to include physical assessment and physiological reasoning qualifications as Masters level)
- Non-Medical prescribing essential
- Recognised teaching qualification or relevant experience
NATURE OF EXPERIENCE
Essential
- Experience working with frail, elderly and or patients with dementia which will include adults from the age of 18 years
- Managerial and clinical leadership experience within a large multidisciplinary organisation.
- Experience of successfully leading clinical teams
- Experience of working with external agencies for the effective delivery of services
- Experience of leading Quality Improvement programmes
Desirable
- Experience working in urgent/acute care in a community setting
- Experience of educating and assessing others in clinical practice knowledge and skills
- Understanding of working in a political environment.
- Detailed working knowledge of how their health community operate and how they fit into the health and social care community
