Job summary

Are you a newly qualified OT or a more experienced Band 5 looking for a post to give you a wide variety of community experience?

Here in the Cotswolds integrated community team we have a unique opportunity for you to develop skills across a wide range of community Occupational Therapy. We provide short term OT interventions, such as the rapid assessment of patients within their own homes to prevent admission or support hospital discharge, and where we differ from other community teams outside of Gloucestershire is that we can also offer you the opportunity to develop your competencies in long term condition management and housing adaptations, including the disabled facilities grant process.

Main duties of the job

We have a very supportive team of OT's who can offer you regular supervision, support with your preceptorship, training and annual appraisal as well as working through our countywide therapy competency framework.

As members of an Integrated team we work alongside community nurses, physiotherapists, Reablement workers and our Adult Social Care colleagues.

In all aspects of our interventions we promote personalised care, working with our patients to identify what matters to them, promoting independence and enabling them to achieve their goals.

We are an active player in the development of the local Primary Care Network entering an exciting time as we work with our partners to develop local place based models of care.

We work in partnership with GPs, community hospitals, mental health services and voluntary organisations to enable people to live as independently as possible at home. The ethos of the teams is a Reablement, person centered approach to recovery and self-management, to ensure we offer care close to home, thus avoiding unnecessary admissions to bed based services.
