Job summary

The Care Home Support Team is seeking to recruit keen and a motivated Senior Community Mental Health Nurse who has a passion for working with people with dementia and their carers to help maintain a good quality of life within the care home setting. The role involves person centred assessments for service users with a diagnosis of dementia living in care home's within Gloucestershire incorporating the '5 step approach', you will be given support and training on the five step approach. The role also has an element of teaching care home staff on various topics including dementia awareness and mental capacity act amongst others.

As a senior member of the team you would be expected to provide clinical and leadership guidance to your team. You will be expected to take a lead on the training aspect for the team along with other members of the team, help maintain requests, work closely with our training partners and to co-ordinate training requests from care home.

Previous experience of working with people living with dementia and working in a community setting is preferred.

Although you will be largely responsible for a set geographical area, you may be expected to provide cross cover across the county to assist with your colleague depending on size of case load and referral rate.

Main duties of the job

The Post Holder will be a senior Member of the team working with individuals across the locality who have dementia and live in a care home setting. The purpose of this role is to:

  • Act as a short term care co-ordinator providing assessments and treatment plans to support individuals and care home staff to improve resident's quality of life.
  • Caseload Management
  • Offer clinical leadership to other members of the Care Home Support Team Countywide.
  • Provide expert clinical advice, support, training to individuals, their families and care home personnel
  • Whilst the post is for a set geographical area the successful applicant may be expected to provide cross cover in other areas of the county
