Delegated Commissioning Placements Manager (Interim)

Job summary

An exciting opportunity has arisen to join an established & well-organised Delegated Commissioning team within Surrey and Borders Partnership NHS Foundation Trust. The team is led by an experienced Consultant Psychiatrist & works closely with clinical services, commissioners & independent sector providers to ensure people with highly complex mental health needs receive appropriate, high-quality care.

The role will manage a caseload of specialist mental health placements for individuals whose needs cannot be met within local services or those commissioned nationally by NHSE. The role involves overseeing placements across a range of providers & ensuring care is delivered in line with national standards, contractual expectations & best clinical practice.

A strong clinical background is essential. The post holder will work with multidisciplinary teams and commissioners to review care pathways, monitor recovery progress & ensure placements remain clinically appropriate while supporting effective care planning and step-down opportunities.

The role requires excellent organisational & communication skills, the ability to manage multiple complex cases & to build strong working relationships with clinicians, commissioners, providers & families to support safe, person-centred & outcome-focused care.

The role will involve visiting provider services, a full driving licence & access to a vehicle for work purposes is essential.

Main duties of the job

  • Manage a delegated commissioning caseload of individuals with complex mental health needs placed in specialist inpatient and community services.

  • Provide clinical oversight and assurance that placements deliver safe, effective and high-quality care in line with national standards and commissioning requirements.

  • Monitor patient pathways and recovery progress to ensure placements remain clinically appropriate and support positive outcomes.

  • Work collaboratively with multidisciplinary teams, commissioners, local authorities and independent sector providers to coordinate care pathways and discharge planning.

  • Lead on the oversight of complex cases, ensuring effective communication between clinical teams, community services and Section 117 partners.

  • Monitor provider performance and quality through placement reviews, provider visits and participation in contract and quality monitoring processes.

  • Ensure safeguarding processes are followed and escalate concerns appropriately in line with Trust and regulatory requirements.

  • Contribute to reporting, data analysis and service improvement to support effective oversight of specialist placements and commissioning arrangements.

Key Responsibilities of the role are:

  • To be responsible to manage the Delegated caseload consisting of inpatient and outpatient cases.
  • To ensure appropriateness of placement for the service user.
  • To provide clinical assurance of the quality of the care provided by specialised services for those funded through the Delegated Commissioning function
  • Managing individual patient pathways to ensure high quality services and care planning outcomes are achieved
  • To ensure there is a coordinated approach to care and discharge planning, which results in robust and effective care packages
  • To take a lead as appropriate in the interface between the Community Forensics Team, Local Authority representatives, and S117 managers in the monitoring and follow up of care pathwaysEnsure services have mechanisms in place for patient and carer involvement in their care plans
  • To build working relationships with care coordinators, CCG and NHS England in order to promote efficient pathways for specifically complex or disrupted care pathways.
  • To assist in the collation of monthly reports and data collection. Reports will be generated to the Trust board, CCG and CMHRS.
  • Ensure that quality is maintained by; regular monitoring of providers, attending regular contract meetings and ensuring the Delegated authority quality monitoring cycle is maintained.
  • To operate a cost effective service that drives continuous improvement and proactively promotes and supports change to deliver excellence in service delivery.
  • To engage proactively with the designated DCT contracts manager to ensure contractual priorities/concerns/challenges are followed up on in a timely manner. To take action in respect of safeguarding Policies and Procedures as required
  • To be aware of the escalation process to regulatory bodies such as CQC
  • Provide and receive highly complex, sensitive and contentious information, presenting information, to a wide range of stakeholders in a formal setting.
  • Supporting the team in the assurance of specialised services compliance to quality, applicable specifications and guidance
  • To support the development and implementation of an integrated approach to business intelligence, combining financial, HR and activity data to create a full picture reporting environment necessary to support effective decision-making in the Trust.

  • Must have an understanding of the background to and aims of current healthcare policy and contracting environment and appreciation of the implications
  • Be willing to obtain detailed operational knowledge of complex services, including risk assessment/management
