Job summary
We are excited to share an opportunity for a Band 7 Clinical Team Lead.
The role involves working in our hub located in Frimley and providing a crisis and home treatment service for all emergency, urgent and routine referrals in the Surrey Heath and Northeast Hants area. We are looking to recruit a Band 7 Clinical Team Lead to be part of and lead the service, providing senior clinical and managerial leadership. You will be accountable for the day-to-day management of the service, overseeing referrals, assessments, allocations and discharges to ensure a high standard of patient care.
Working closely with the Service Manager, you will drive service governance through audit, quality-improvement initiatives and performance-data audit, and ensure compliance with CQC standards.
You will support the management of operational functions such as incident investigations, complaints and escalations. You will provide supervision, appraisals and professional development for the team, and oversee recruitment, induction and training.
You will collaborate with inpatient wards, community mental health teams, primary care, social services and voluntary partners to ensure seamless care pathways and drive service improvement.
This is an integrated role to support the triage and/or rapid response as well as the Home Treatment elements of the service and the post holder will be responsible for creating an environment which is safe, supportive and an easily accessible with a no wrong door approach.
Main duties of the job

- Discharge professional responsibilities under the Mental Health Act (1983) and revised subsequent act Mental Capacity Act (2005) and the NMC Professional Code of Conduct
- Be accountable and responsible for own practice.
- Develop, implement and evaluate evidence-based programmes of care; set standards of care and liaise with other agencies, particularly in relation to people discharged under the Care Programme Approach.
- Participate in the multi-disciplinary assessment of individuals with mental health problems or vulnerabilities living in the community, by providing a community mental health perspective and expertise
