Job summary
My job makes better lives by ensuring an accessible, non-stigmatising, localised service that contributes to the continuing care and rehabilitation of people within the Guildford and Waverley locality with established mental health illness/dementia living in their home and/or a care home.
Job Overview
To work within a community mental health team or a care home pathway based within Guildford and Waverley area. The successful candidate will be working in partnership with statutory and voluntary sector to facilitate whole system approach to care delivery. The Post Holder will act as Care Coordinator/Lead Professional, where appropriate, to people living in their own homes and care homes with mental health diagnosis and/or a dementia, who require the skills of a Mental Health Nurse, who can provide a holistic, clinical autonomous nursing service, to enable individuals to engage in the best standards of care available.
The delivery of care should encompass NSF standards and Government Policy guidelines. To assist in improving the quality of life, for older people with mental health problems, and enable them to remain in their own home for as long as possible.
Main duties of the job
- To deliver a high-quality service to older people with a mental health diagnosis and/or a dementia within a multi-professional framework.
- To develop shared understandings of people's presentations that help people to better comprehend the underlying function(s) i.e. behaviour that challenges.
- To act as key worker/care co-ordinator for named individuals and adhering to the Care Programme Approach process.
- The ability to work independently, autonomously and take personal responsibility/initiative.
- To provide high standards of care, which are delivered in a professional manner.
- To ensure that the care delivered is based on best practice and is regularly revised.
