Job summary

This role is based within our Care Home Pathway. You will be working alongside colleagues from a range of professional backgrounds (psychology, occupational therapy, nursing and associate nurse practitioners) and the Surrey Heath Integrated Care Team to provide specialist advice and consultation to people with dementia living in care homes within the local area. You will be assisting people who are displaying behaviours that challenge themselves and/or others by drawing on ideas from a Positive Behaviour Support framework. You will also be supporting the CMHTOP in memory and functional assessments for patients in Care Homes.

This is a senior role within our Care Home Pathway; you will be expected to provide leadership to the team, including supervising and line managing the Care Home Pathway junior staff.

Main duties of the job

Your responsibilities will include completing assessments with a wide range of people, helping teams to understand behaviour that challenges and sharing ideas to promote wellbeing. You will have skills in writing care plans for people with dementia and/mental health disorder and sensitively sharing these ideas with staff teams. You will possess the dementia knowledge and skills to work at a high level of competence within agreed protocols and with clearly defined lines of accountability for delegated work. Finally, you will be prepared to role-model ideas (including the provision of personal care where needed) and also be competent in negotiating IT systems (such as SystmOne).

Job responsibilities

To offer high quality memory assessments to people with comorbid physical and mental health needs, and often with frailty or delirium under the care of the Surrey Heath CMHTOP Care Homes Pathway. These may include people that are housebound and living in care/nursing homes.

To provide individualised short-term mental health interventions as necessary to the above cohort of patients as necessary.

To assess those with cognitive impairment and (following Clinical Supervision) provide diagnosis if identified and post-diagnostic support to individuals and their families.

To work with staff within the Surrey Heath integrated care teams and care homes to increase awareness and recognition of mental health/dementia issues affecting adults and their families/carers.

To develop and deliver education packages, which will support colleagues in the management of patients with dementia and/or challenging behaviour and mental health issues within Surrey Heath.

To develop systems whereby Integrated Care & Care Homes staff are confident in their ability to filter out and manage the less complex patients, referring only those with the most complex dementia and or mental health needs to the Care Homes Pathway for assessment and further treatment.

To provide expert support for patients and their families/carers who are being managed within the SH CMHTOP Care Homes Pathway and require specialist mental health support.

To provide expert consultation, liaison, signposting and support for other professional within the SH CMHTOP CHP team.

To work with colleagues to develop and manage integrated health and social care patient pathways that bridge primary, secondary, and acute in-patient care and offer alternative options for support to both patients and carers within the Care Homes Pathway.

Develop strong links with Liaison Psychiatry and acute hospital colleagues, supporting the timely discharge of people from acute settings whose mental health needs would be better met within the Care Homes Pathway.

To work in conjunction with primary care staff in facilitating the Delirium Pathway, whilst supporting the people who use our services, their family and carers to promote the best possible outcome.
