Job summary

We are seeking a Clinical Lead to work within the Mid Community mental health team for older people based in Epsom. We are a committed and passionate group of professionals, dedicated to meet the needs of people with a range of diverse and challenging difficulties.

This post is suitable for someone who is passionate, dynamic, and committed. The applicant should be able to work well within a team will have the necessary qualifications, experience and skills to manage their caseload to a high standard , delivering a high - quality service to those under our care and their families. This vacancy would suit a Registered Mental Health Nurse, HCPC Registered Occupational Therapist with two years post qualification experience in working in a Community Mental Health Setting.

MID CMHTOP is a is a multi-disciplinary based community mental health service and aim to provide evidence based treatment and interventions to people with severe and enduring mental health problems with a focus on Recovery and promoting independence. We are working towards providing treatment within structured Care Pathways that ensure the best outcomes for our client.

Main duties of the job

Facilitate pro-active links and relationships between the Team and the wider community giving advice to fellow professionals ,carers , voluntary organisations and the public.

- Foster collaborative working relationships with the whole system

- Ensure appropriate and timely transfer of care between services

- Ensure that all professionals maintain accurate databases

- Facilitate regular review of caseloads, providing guidance and support to ensure all care is of a high quality and is evidence based .
