Learning Disability Liaison Nurse for Primary Care

Job summary

  • Identifying people with a Learning Disability on the GP QOF register
  • Promoting annual health checks, for adults aged 18 and over with our Primary Health Care partners.
  • Providing education and training, in line with DES contracts.
  • Advocate for reasonable adjustments.
  • Develop and maintain good working relations with our partners, with a particular focus on GP practices.
  • Hold a time limited complex health care case load.
  • Lead on specific health related initiatives

Main duties of the job

  • To provide supervision and contribute towards appraisal of junior team members
  • To work as an autonomous practitioner and to escalate any concerns as appropriate regarding the healthcare needs of people with a learning disability.
  • To advocate and represent the service.
  • To contribute and lead on the development and implementation of appropriate policies, protocols, and care
  • Assist in staff recruitment and lead on the induction of new staff
  • To provide support to the service management team as appropriate
  • To demonstrate specialist nursing skills in supporting the needs of the people with a learning disability. This would be in response to complexity, urgency, and planned interventions.
  • To actively involve people in developing and agreeing their own care plan, to act as an advocate as required
  • To carry out comprehensive assessments, plan implement and evaluate delivery of nursing interventions as part of a holistic approach working in a variety of
  • To practice within the Positive Behaviour Support framework.
  • To undertake comprehensive risk assessments and develop effective risk management plans.
