Job summary
We are looking to recruit a motivated, committed crisis response and home treatment mental health nurse practitioners to primarily work night shifts (8pm-8am) for the East and Mid Surrey Home Treatment Service.
You will also be working alongside our night shift nurse and their shift begins at 8.00pm - 8:00am. The post holder will work on a rota including weekends. The role is full-time, however part-time and flexible options are also available.
This opportunity is open to registered mental health nurses, occupational therapists or social workers.
The post holder will be an autonomous practitioner who can also work effectively as team player. You will need to demonstrate excellent communication skills and be able to work collaboratively within a multidisciplinary team. You will be committed to the delivery of a high-quality service, working both independently and as part of a well-established, innovative and crisis response and home treatment team in meeting the needs of service users and their carers.
Qualified clinicians (NMC) in our Home Treatment Teams are currently benefiting from a local retention premium of between £2,840 and £3,425 per annum (based on whole time), dependent on payscale point.
The candidate must have a full and valid driving licence and access to a vehicle for work purposes due to the community element of this role.

Main duties of the job
- Develop, implement and evaluate evidence based programmes of care; set standards of care and liaise with other agencies, particularly in relation to people discharged under the Care Programme Approach.
- Participate in the multi-disciplinary assessment of individuals with mental health problems or vulnerabilities living in the community, by providing a community mental health perspective and expertise.
- Ensure that a risk assessment is completed for service users, especially if there is a history of, or a potential for risk.
- Adhere to professional confidentiality standards
- Be aware of the potential for vulnerability to abuse amongst the people who use this service, and use of the local policy to inform appropriate personnel in line with safeguarding vulnerable adults and children and SABP Policies.
- Ensure that a carers assessment is offered to carers of people who use services, in line with Standard 6 of the National Service Framework.
- Help develop and maintain effective joint working arrangements with colleagues in the community mental health services, residential services, primary health care, social services and voluntary services, thus facilitating the co-ordination and comprehensiveness of provision
