The Surrey Heath Community Mental Health Recovery Service
(CMHRS) is looking for a band 6 Registered Mental Health Nurse to join them.
The Community Mental Health Nurse will be based at Theta in Frimley with the
working age adults service and will work within a multidisciplinary team to
deliver secondary mental health care to the local population.
The Surrey Heath CMHRS is based at Theta which is a light
and modern working space with ample free parking and easy access to local
amenities located within walking distance. The Theta hub hosts a number of
community-based services which supports integrated partnership working.
We are a small, supportive multidisciplinary team made up of
colleagues from a number of professional disciplines including Social Work, Nurses,
Occupational Therapy, Psychologists, Medics and Administrators, every member of
the team holds equal value and hold the shared vision to provide compassionate
and safe care.
Main duties of the job
The role of the Community Mental Health Nurse is wide-ranging
and is an ideal opportunity for someone who is self-motivated, organised and
skilled in providing a range of therapeutic interventions.
Key aspects of this
role include:

Being an active and engaged member of the nursing
team supporting the delivery of key nursing roles and interventions including
physical health monitoring and medicines management.

Working collaboratively with individuals
accessing our services and partner agencies to determine needs and risks and
the appropriate ways to manage these and meet recovery goals.

Contributing to the teams duty rota to provide
a rapid response to individuals in crisis.

Working as a lead clinician or Care Coordinator
to an identified caseload of individuals accessing our services.

Supporting the learning and development of
junior staff through supervision, training/education opportunities and
mentorship.

- To lead the clinical management of a care package to an
individual who uses our services.
- To co-ordinate and monitor the delivery of person centered
healthcare to a designated group of people who use our services, using
complex clinical decision making where appropriate.
- To deliver individual and group therapy utilizing CBT skills.
- To work as a member of the MDT in assessing and planning care
and responding to changes in people who use our services health.
- To ensure provision of the necessary physical care to help
people who use our services i.e.: personal hygiene, diet and fluid intake,
clothing and laundry, personal environment, manual handling, elimination,
maintaining rest and sleep and activity, physical observations.
- To act in a manner to respect the customs, individuality,

relationships with all members of staff with other professionals and
agencies to enhance recovery.
