Job summary

This is a great opportunity to join our Hope Team, which is a flagship service within the Children and Young Peoples Division at SABP. The Service works with Young People between the age of 11 - their 18th birthday. You can either be based in our Epsom or Guildford site. The Hope Service is unique as it is a true partnership between health, social care and education. They provide a Day Programme as well as outreach to meet the needs of our young people. You would have clinical support from senior nurses as well as the Hope Service Consultant Nurse and an opportunity to explore your career pathway and ambitions for the future.

The role is a development role - entry would be at the Band 6 level and progressing to a Band 7 level at your pace. We offer training, which includes for the crisis pathway services, external courses with CPD points regarding assessment and risk assessment of adolescents in crisis as well as DBT-A and DBT skills courses. We would ensure through supervision and appraisal that your PDP is tailored to your interests, areas of development and aspirations.

Main duties of the job

My job makes better lives by leading and managing timely interventions that contribute to the care of children and young people (CYP) with moderate to severe mental health problems and their families.

Job Overview: To be a core member of the multi-disciplinary/multi-agency Hope Service, participating in the response and provision of crisis/urgent assessments, care planning, risk assessing and developing a risk management plan and treatment plan of CYP requiring support for emotional, mental health and social care needs. Often this response will be to facilitate a diversion from T4 admission or out of area placement and allow for recovery as the C/YP remains in their home/placement/community/school.

The Hope Service will provide you an opportunity to work alongside health colleagues such as Consultant Psychiatrists and Psychologists to Social Care Staff such as Social Workers and of course Teachers. Our young people may be attending the day programme due to concerns about family/placement breakdown or the risk of hospital admission. We also offer short term crisis outreach work where the opportunity to work with young people in their communities including school is available.

When our young people are admitted to an adolescent inpatient unit you will find yourself involved in the ward reviews and discharge planning meetings and this year will see the opening of our General Adolescent Unit within Surrey

Job Purpose

Report to the General Manager and Clinical Nurse Lead, and contribute in the development of the health pathway and delivery of service.

To provide timely assessment of CYP who are referred and who may require diversion from T4 admission or out of area placement or who need T4 admission or a placement to meet their biopsychosocial needs.

To provide support, advice and education to the Emergency Departments, Paediatric Wards, and other emergency service provision such as the Police to build knowledge and skills of staff within these settings.

To work individually and jointly within the Mindworks alliance to provide a specialist, assessment and treatment service to CYP and their families/carers in crisis. This may include assessment and treatment within a community, acute hospital of HBPoS (Health Based Place of Safety) setting.

Band 6 Key Responsibilities

The post holder will work closely within the Mindworks alliance during office hours, responding to crisis and urgent referrals of CYP or for crisis and urgent presentations of CYP already working with the Hope and Ext Hope Services.

The post holder will work as an autonomous practitioner, undertaking clinical assessment of CYP who may have a complex co-morbid mental health needs and present in crisis, working alongside medical, nursing and other professional colleagues to ensure that the most effective care and treatment is provided to meet their current needs.

Work closely with the Emergency Departments, Crisis Intervention Service, Extended Hope, CCISS, the Adult Psychiatric Liaison Service, other Mental Health Crisis/urgent Teams, On Call Duty Psychiatrist, On Call Managers, Paediatric Liaison Nurses, ensuring CYP are seen in a timely manner and risk is assessed and a management plan is developed.

To work within a highly experienced multi-disciplinary team involved in the assessment and treatment of CYP with serious and complex mental health difficulties in crisis.

To manage own caseload from referral to discharge/transfer, holding medium to high-risk cases and ensure timely and accurate documentation is completed.

Ensure that outcome measures for the Hope Service are implemented and audited. Contribute to relevant audit/research and ensure that interventions on the pathway are evidence based.

Interface with other services/agencies regarding care, treatment and discharge planning.

Band 7 Key Responsibilities (in addition to Band 6)

The post holder will work closely with the Mindworks alliance during office hours, responding to consultations or response to referrals received for possible Hope Service involvement:

Report to and advise referrer on assessment and treatment outcomes and respond to requests for and provide formal reports for other agencies.

Represent the Hope Service at professional/network meetings to offer advice/consultation on complex cases who are in crisis.

Collaborate on developing systems to monitor and evaluate the service

Supervise other junior members of the team and contribute to their appraisal process.

To undertake relevant CPD to improve assessment and treatment skills.

Be competent in conducting assessments in all settings.

Deputies for the Clinical Lead Nurse where required

Other duties:

To actively formulate a robust crisis and contingency plan aka My Safety Plan with CYP and their families/carers inclusive of risk assessment and management and positive risk taking.

Collaborate on the development and lead on the implementation of clinical guidelines, protocols relevant to sphere of responsibility.

Development Expectations: Overview.

The person will be allocated a local mentor to support them in the first 6 months. The following is an outline of the development expectations over time for the post. However, it is not exhaustive, and timeframes can be negotiated.

The clinical and managerial supervision process within the Hope Service will monitor and tailor the process to each individual and will monitor progress after 6 months.

It is expected that by the end of the 6 months - 2-year development period the person will be able to demonstrate meeting the required competencies at a Band 7 level as outlined in the above competency profile and that the person has taken direct responsibility for that development or communicated difficulties in doing so via the support systems provided.
