Job summary
This is a great opportunity to join our Hope Team, which is a flagship service within the Children and Young Peoples Division at SABP. The Service works with Young People between the age of 11 - their 18th birthday. You can either be based in our Epsom or Guildford site. The Hope Service is unique as it is a true partnership between health, social care and education. They provide a Day Programme as well as outreach to meet the needs of our young people. You would have clinical support from senior nurses as well as the Hope Service Consultant Nurse and an opportunity to explore your career pathway and ambitions for the future.
The role is a development role - entry would be at the Band 6 level and progressing to a Band 7 level at your pace. We offer training, which includes for the crisis pathway services, external courses with CPD points regarding assessment and risk assessment of adolescents in crisis as well as DBT-A and DBT skills courses. We would ensure through supervision and appraisal that your PDP is tailored to your interests, areas of development and aspirations.

Main duties of the job
My job makes better lives by leading and managing timely interventions that contribute to the care of children and young people (CYP) with moderate to severe mental health problems and their families.
Job Overview: To be a core member of the multi-disciplinary/multi-agency Hope Service, participating in the response and provision of crisis/urgent assessments, care planning, risk assessing and developing a risk management plan and treatment plan of CYP requiring support for emotional, mental health and social care needs. Often this response will be to facilitate a diversion from T4 admission or out of area placement and allow for recovery as the C/YP remains in their home/placement/community/school.
The Hope Service will provide you an opportunity to work alongside health colleagues such as Consultant Psychiatrists and Psychologists to Social Care Staff such as Social Workers and of course Teachers. Our young people may be attending the day programme due to concerns about family/placement breakdown or the risk of hospital admission. We also offer short term crisis outreach work where the opportunity to work with young people in their communities including school is available.
When our young people are admitted to an adolescent inpatient unit you will find yourself involved in the ward reviews and discharge planning meetings and this year will see the opening of our General Adolescent Unit within Surrey
