Would
you like to become a CPN at NW Community Mental Health Team for Older People,
(Spelthorne Locatlity)?
Based at Unither House in Chertsey, the NW CMHT for Older People’s service offers assessment,
diagnosis, treatment and support for people over 65 who present with functional
mental health illnesses; deterioration in cognition and/or dementia. We work
closely with other services, such as social care and also with the voluntary
sector, particularly the Alzheimer’s Society, to provide positive support for
people using services and their carers, ensuring effective forward signposting
where appropriate.
We are looking
for a passionate nurse who will focus on providing support and high quality
nursing care for clients and carers, targeting and prioritising people with
severe mental health problems to join the NW Community Mental Health Team for
Older People.
Main duties of the job
You will be required to
demonstrate a high level of commitment, motivation and knowledge working with
people with severe and enduring mental health problems and/or dementia. You
will also need to demonstrate clinically effective care management skills and
interventions in accordance with the Care Programme Approach.
You will manage a caseload of
clients, and use evidence based/client centred principles to assess, plan,
implement and evaluate interventions as part of the holistic approach. You will
need to demonstrate excellent communication skills and be able to work
collaboratively with a multi-disciplinary team. You will be committed to the
delivery of a high quality service, working both independently and as part of a
well-established and innovative team in meeting the needs of clients and their
carers.

The Post Holder
will act as care co – coordinator, where appropriate to patients who require
the skills of a Mental Health practitioner, and provide a holistic, clinical
autonomous mental health service, to enable individuals to engage in the best
standards of care available.
The Post Holder
will work within a community team based within the North West Surrey sector
working in partnership with statutory and voluntary sector to facilitate whole
system approach to care delivery.
The delivery of
care should encompass NSF standards and Government Policy guidelines. To assist
in improving the quality of life, for older people with mental health problems,
and enable them to remain in their own home for as long as possible.
The Post Holder
will support Primary Care in management of people presenting with mental health
problems by offering assessment, diagnosis and treatment options.
The Post Holder
will liaise with Primary Care professionals, through regular informal and
formal meetings.
Principal Duties
MAIN DUTIES AND RESPONSIBILITES :
- To
deliver a high quality community mental health service to Older People
with mental health problems, in a multi – professional framework.
- To
prescribe services that meet the user / carer needs, in conjunction with
others.
- To
act as key worker / care co-ordinator for named individuals, within the
Care Programme Approach process, including Single Assessment
- The
ability to work alone
- To
provide high standards of care, which are delivered in a professional
manner.
- To
ensure that the care delivered is based on best practice, and is regularly
reviewed.
- To
attend and ensure all referrals are passed through CMHT allocation
meeting, and assessed for acceptance to the specialist team.
- To
ensure all records and reports are kept up-to-date, and up to the standard
expected of this grade of nurse, in accordance with the Caldicott Report.
- Keep
an accurate, up-to-date diary and record of visits.
- Be responsible
for the continuous overall management of a defined caseload.
- To
receive and give clinical and management supervision / guidance
- To
supervise juniors / other staff in the delivery of care and treatment to
individuals and groups
- To
support junior staff to manage caseloads effectively

act as a positive role model for junior staff, assisting / guiding them in
their duties
