Job summary

An exciting opportunity has arisen for a passionate and committed professional to join the East Surrey Community mental health team for older people as a Band 7 advanced practitioner. The team cover the areas of Horley, Redhill and Tandridge.

The post holder will have the necessary qualifications, experience and skills to manage their caseload to a high standard, delivering a high-quality service to those under our care and their families.

Main duties of the job

As an Advanced Practitioner you will be responsible for a defined complex caseload within the team. Using specialist Mental Health knowledge and skills, carry out individual clinical assessment, plan and formulate a range of interventions and treatment options within care plans, implement and evaluate packages of care.

You will undertake the task of care coordinator, organise CPA review meetings, discharge planning meetings involving service users, their carers, other professionals and agencies, attend inpatient reviews and act as client advocate if necessary.

The successful candidate will facilitate the development and use of care pathways by the multi-disciplinary team and provide supervision to junior colleagues.

The post holder will be responsible for providing senior clinical support to the duty system and will also be expected to work as frontline practitioner on duty.

The post holder will lead on the setting, monitoring and maintaining of standards of care within the team and will advise the Sector manager on clinical issues which may have a negative effect on the standards of clinical practice being delivered.

The post holder will be responsible for their own practice in accordance with their professional body and will take every reasonable opportunity to sustain and develop their professional competence.

Job Overview

To ensure the service operates in a safe, effective & efficient way as part of the whole system of mental health services.

In addition, to foster a culture that encourages and enables individuals to have an active role in identifying, planning and meeting their individual needs.

To support strategic leadership in relation to service improvement and quality evaluation.

The Community Mental Health Team for Older People offers comprehensive mental health support and treatment to people who present with mental health and/or memory problems. There are three main functions:

1. Promotion of Mental Health and social inclusion. This is achieved through individual work and clients as well as through assessment, treatment and provision of therapeutic GP work.

2. Provide Mental Health support, training and advice to staff and in local Day Centres and Wellbeing Centres with partnership in Surrey County Council, Carers, GPs, Registered care homes and Voluntary sector.

3. Provide skilled Mental Health assessment and treatment to people in the community, and continuity if they are admitted to Acute mental health hospital wards. Teams work with people if moderate to severe needs.

The post holder will be responsible for providing clinical and managerial supervision to Band 6 clinical staff and junior staff, where appropriate.

The post holder will provide specialist mental health advice and education to primary care colleagues.

The post holder will provide holistic, autonomous care and maintain communication with primary care professionals and other professionals and agencies through regular informal and formal meetings.

The post holder will be responsible for care coordinating a caseload of people presenting with complex mental health needs.

They will undertake initial assessments on those presenting with more complex needs and be responsible for the development and delivery of care plans to meet any identified needs.

Have a minimum of 5 years experience working with people with MH needs, 2 of which must be at a Band 6 with Community Mental Health experience.

Establish and embed a senior leadership team function for the service.

Responsible for the development of a high-quality service

Lead & motivate staff to provide a pro-active and responsive high-quality service, contributing to ongoing audit and evaluation.

Ensure that the necessary systems & protocols for the delivery of a safe & supportive service are in place, understood & adhered to throughout the Service.

Ensure that effective communications systems are established, maintained and monitored regularly within the Service & between the Teams & other services/agencies/partners.

Provide line management, for staff within the Service.

Ensure arrangements are in place for all service members to receive regular supervision, appraisal and have their personal development plans and training needs identified and where possible met.

Access and use information contained within the Trusts patient based information systems.

Lead and be responsible for ensuring that they and all team members fulfil our duties regarding Safeguarding Adults.

Key Practice Issues and Responsibilities:

Facilitate pro-active links and relationships between the Team & the wider community giving advice & guidance to fellow professionals, carers, voluntary organizations & the public.

Foster collaborative working relationships with the whole system.

Ensure appropriate and timely transfer of care between services.

Ensure that all practitioners maintain accurate databases.

Facilitate regular review of caseloads, providing guidance and support to ensure all care is of a high quality and is evidence based.

Leadership:
