Job summary
To work as a fully integrated member of the multidisciplinary team providing direct care to people using our services.
Develop positive relationships with the people who use our services, and their carers, adopting a pragmatic and non-judgemental approach and encourage and support individuals to participate in ways which support their individual recovery journey.
Contribute to meaningful care plans and risk management plans and deliver low intensity therapeutic interventions.
Main duties of the job
Key Responsibilities:
- Assist in all areas of care in accordance with agreed care plans and procedures to enable people using services to meet their individual needs.
- Conduct, record and report observations, and ensure care plans are implemented and quality standards achieved.
- Monitor the wellbeing of people using servicesActively engage people using services, particularly those on higher level of observations and with greater care needs, in therapeutic conversation and activities.
- Undertake physical health checks, i.e. Blood Pressure, Temperature, Respirations, CPR and use of Defibrillator
- Explain principles, techniques, and safety procedures to participants in activities and demonstrate use of materials and equipment.
- Encourage and support individuals to take part in activities suited to their particular needs and interests.
- Provide and promote recovery-orientated support and low intensity therapeutic interventions to people using services in a way that respects their views, autonomy and culture.
- Attend and contribute to meetings, ward reviews and care programmes meetings as appropriate.
- Assist in assessing of risk and developing care plans. To adhere to the requirements of the Trust policies and procedures at all times.
