Job summary

My job makes better lives by leading and managing care of Children and Young People (CYP) who present in the Emergency Department (ED) at the District General Hospital (DGH) and by working in partnership with the multi-disciplinary team consisting of representatives from the ED, police, ambulance, and mental health service alongside CYP and their families/carers collaboratively making a difference to their lives.

Main duties of the job

As part of a partnership within the Paediatric ED at the DGH it is vital to ensure the service operates in a safe, effective and efficient way as part of a whole system. Foster a culture that encourages and enables individuals to have an active role in identifying, planning and meeting their individual needs. Support strategic leadership in relation to service improvement and quality evaluation.

  • Lead on the setting, monitoring, and maintaining of standards of care and advise on clinical issues which may have a negative effect on the standards of clinical practice being delivered
  • Responsible for your own practice in accordance with your professional body and you will take every reasonable opportunity to sustain and develop your professional competence
  • Be a mentor and advisor for students and will work closely with the Clinical Placement Facilitator to ensure that the learning objectives are consistent with the requirements of their educational programs
  • Provide clinical leadership for a range of clinicians in their specific area of practice, providing advice on all clinical issues within that service; support clinicians with updating their relevant knowledge and skills

Job summary

  • To provide visible, credible professional clinical leadership, supporting the clinical management of CYP with mental health issues who have presented to the Emergency Department, who may have CYP with challenging behaviour as a result of an underlying condition such as learning difficulties, ASD and ADHD.
  • To improve patient experience, quality and efficiency of service in DGH by focusing on the significant number of CYP with mental health issues that can complicate their care and discharge.
  • Ensuring the care delivered to CYP with mental and physical health needs is of a high quality, research based, and is CYP and family/carer focused.
  • To support the wider paediatric nursing and medical team, by arranging training opportunities to up-skill all professionals on effectively managing CYPs mental health needs.
  • To manage CYP presenting with mental health. Emotional health and social care needs, co-ordinating between the medical, nursing CYPS community teams.
  • Be a champion for CYPs mental health and an effective role model for other team members supporting and promoting a learning environment.
  • Work in close liaison with the Paediatric Matron/staff to allocate safe and appropriate special nursing care to CYP as required.

Clinical Duties

  • To clinically undertake mental health/risk assessment, which can be used to inform the care planning of the CYP whilst they are at DGH.
  • To maintain accurate contemporaneous records during assessments, complying with the trust documentation policy and that of your professional body
  • To have excellent verbal and written communication skills so that professional advice or care guidance and instructions are well understood by CYP, their family/carers and members of the multidisciplinary team. This may involve imparting information that may not be well received by the CYP and their family/carers
  • To manage clinical situations that may be challenging and distressing for staff , ensuring the needs of the CYP and family/carers are paramount
  • To ensure effective discharge planning so that individual CYP needs are met with an appropriate plan of action involving the multidisciplinary team as required
  • Function as an effective role model in the clinical setting in demonstrating the application of breadth and depth of knowledge and skills pertaining to mental health in CYP, and in advising and supporting members of the multi-disciplinary team.
  • Support and mentor junior colleagues
  • To work collaboratively with the Manager/Matron to ensure that clinical governance processes at unit level encompass measures to ensure the quality of the fundamental and essential aspects of care are met.
  • To improve the experience for CYP presenting to the Emergency Department.
  • Ensuring the nursing care delivered to CYP with mental health, emotional, physical health and/or social care needs, is of a high quality, research based, and is CYP and family/carers focused.
  • To foster a close working relationship with clinicians and Allied Health Care Professionals to ensure a multidisciplinary approach to care in line with clinical governance
  • To communicate sensitive information to CYP and their families/carers. To respond sensitively to the needs of relatives/carers.
  • To develop strong relationships with the Mindworks Partnership (aka CAMHS community provisions), as well as other statutory and non-statutory agencies.
  • To assist with the management of the behaviour of CYP who at times may be physically and verbally aggressive towards other members of family/carers and staff.
  • To participate in responding to crisis referrals including consultation, assessment, monitoring of mental state and medication side effects, liaising closely with clinicians/teams involved with the CYP. To be able to utilise skills flexibly so that interventions can be planned and implemented for CYP presenting in the Emergency Department.
  • To conduct accurate risk assessments (from admission to discharge) and management plans, involving CYP. Families/carers and other agencies involved with the CYP.

  • To provide consultation to other professionals within CYPS.

Management Responsibility/Professional Leadership

  • To effectively manage the pathway from admission until discharge for CYP
  • To assist in the decision-making regarding skill mix and presenting acuity and the need for 1:1 staffing
  • To work alongside and support staff when a CYP presents in crisis or displaying dysregulated behaviour towards themselves or others and work to support that young person to de-escalate
  • Provide positive effective leadership and act a role model. Delegate appropriately and empower staff to problem solve effectively with their daily clinical workloads
  • Facilitate and support nurses in working collaboratively with all other members of the multi- disciplinary team
  • Use of leadership styles appropriate to the changing circumstances/landscape

  • To undertake a recognised assessor training and subsequently participate in the formal assessment of learner nurses (nurses only).
  • To plan and maximise opportunities to deliver work-based learning for the Emergency Department Team. This may extend to the Paediatric Ward.
  • To develop education and training programmes in collaboration with Paediatric Matron to ensure that knowledge, skills and competencies of the trained staff and students are able to meet CYP, families/carers' by develop learning outcomes and competency assessments for all staff
  • On a monthly basis to undertake a regular teaching/training shift to either deliver education or work clinically with a junior member of the nursing team to gain confidence knowledge and skills in an agreed subject
