Emotional Wellbeing Mental Health (EWMH) Services and into appropriate mental
health or community services as they reach 18 years of age?
We have an exciting opportunity for 2 Support Workers to join
Surrey & Borders NHS Foundation Trust (SABP) in the newly transformed
Reaching Out service. The Reaching Out
service focuses on engaging and supporting the most isolated and vulnerable
children and young people through a multi-agency network approach. You will be a part of the Transition Pathway,
which provides support for 17 and 18-year-olds as they transition out of
Childrens EWMH Services.
We are looking for clinicians who are creative, highly
motivated, shows initiative and is passionate and committed for ensuring high
quality services for people with mental health needs.
There are 2 posts, one covering the South West area of Surrey and one covering the South East area of Surrey.
Main duties of the job
You will case manage and co-ordinate support for young
people aged 17-18 years old as they transition, developing a Transition Care
Plan with the young person to identify their needs and ensure they offer the
right help and support throughout their transition period and beyond.
You will accompany the young person to appointments,
assessments and therapy, educate and reflect on the differences in culture and
practice between services, as well as help to signpost and/or refer to services
which will be emotionally beneficial for the young person.
You will be required to attend regular multi-disciplinary
team meetings and liaise with a range of professionals, agencies and other
parties involved in the young persons care.
A key part of this role is building relationships and trust
with young people, being comfortable having conversations and persevering with
a young person about their needs, worries and resistance to engage.
This role is suitable for someone who has excellent
knowledge of the mental health and community services that are available for
young people across Surrey for young people.

in culture and practice between services, as well as help to signpost and/or
refer to services which will be emotionally beneficial for the young person.
The successful candidate will be required to attend regular MDT meetings and
liaise with a range of professionals, agencies and other parties involved in
the young persons care.

Approachable, friendly and have the ability to engage and
build relationships with young people with complex mental health needs.
Being able to think creatively about how to engage with a
young person.
Being a consistent and reliable advocate/voice for a young
person during the transition period.
Accompany a young person to appointments, assessments,
meetings, and therapy as required.
Be able to develop an understanding of a young persons
needs/challenges and signposting to appropriate mental health and community
services to support that young person.
Assisting a young person to build support networks to help
them maintain their independence in the community.
To liaise with colleagues and other professionals or
agencies as required, including attending MDTs, planning meetings, transfer of
care meetings etc.
Confidence to contribute to MDT discussions around plans
for a young person.
Having a good understanding and be able to work closely
with mental health professionals.
To hold an individual caseload under the guidance of a
qualified member of staff.
Being able to work with families in a way which is helpful
to that young person.
Being comfortable lone working with a young person.
Must hold a current driving license and have access to a
car for work purposes.
IT skills including ability to use a computer.
Ability to accurately and concisely document notes and
risks onto system after sessions.
Ability to conduct risk assessments, identify changes to
clinical risk and implement actions to ensure a persons safety.
Knowledge of responsibilities in relation to the
Childrens Act.
Knowledge and awareness of Safeguarding procedures and the
experience of developing a plan of care to safeguard an adult or child.
