Job summary

We are keen to recruit an experienced Band 7 Community Psychiatric Nurse to provide specialist input to the Care Home Pathway within the team. You will be working alongside colleagues from a range of professional backgrounds (Psychiatrists, Psychology, Occupational Therapy, Nursing, Assistant Psychologist and Associate Practitioners) to provide specialist advice and consultation to people living in care homes within the local areas.

This will predominantly focus on people with dementia who are showing behaviour that challenges themselves or others and may also include people with moderate-severe mental health difficulties.

Main duties of the job

You will provide holistic, autonomous care and maintain communication with primary care professionals and other professionals and agencies through regular informal and formal meetings. These meetings will include a review of patients within the practice/service with mental health problems.

You will facilitate clinical sessions and assess/triage clients/patients according to agreed assessment tools, offering short-term interventions based on psychological therapies and offer professional advice to client/patients and their carers, other professional, statutory and voluntary agencies. Additionally, you will also provide advice and guidance to care homes through the established Care Home Pathway Advice and Guidance Service.

You will facilitate the transfer of patients between primary and secondary care and between the different specialist teams within the Trust and care coordinate clients on both standard and enhanced CPA.

You will lead on the setting, monitoring and maintaining standards of care and will advise the Sector Manager on issues which endanger standards of practice within your specific area of practice.

Job Overview

· To ensure the service operates in a safe, effective and efficient way as part of the whole system of mental health services.

· In addition, to foster a culture that encourages and enables individuals to have an active role in identifying, planning and meeting their individual needs.

· To support strategic leadership in relation to service improvement and quality evaluation.

· To oversee and manage a service, including advice and guidance, dedicated to individuals residing in care homes with functional and organic diseases or a combination of both.

The clinical Lead’s post and other Band 7 Clinical Leads will need to cover for each other as needed, such as annual leave, training or sick leave.

The post holder will be responsible for providing clinical and managerial supervision to Band 6 clinical staff and junior staff, where appropriate.

The post holder will provide specialist mental health advice and education to primary care colleagues.

The post holder will provide holistic, autonomous care and maintain communication with primary care professionals and other professionals and agencies through regular informal and formal meetings.

The post holder will be responsible for care coordinating a caseload of people presenting with complex mental health needs.

They will undertake initial assessments on those presenting with more complex needs and be responsible for the development and delivery of care plans to meet any identified needs.

Qualifications required:

· A clinical qualification to degree level or equivalent.

· Membership of a professional body (NMC)

Experience required:

Have a minimum of 5 years’ experience working with people with Mental health needs and care home experience, 3 of which must be at a Band 6 with Community Mental Health experience.

Suitable for someone who is passionate and committed and who enjoys working with people living in Care Homes, managing staff well-being and supporting people to achieve their potential.

Key Responsibilities:

· To promote positivity, can-do attitude and to enhance the experience of the team members and the people we serve.

· Establish and embed a senior leadership team function for the service.

· Responsible for developing and implementing strategies that promote an environment of involvement and recovery.

· Ensure that the agenda of social inclusion is reflected in interventions offered and implemented.

· Responsible for the development of a high-quality service.

· Ensure that effective communications systems are established, maintained, and monitored regularly within the Service, between the Teams and other services/agencies/partners.

· Provide line management, for staff within the Service.

· Ensure arrangements are in place for all service members to receive regular supervision, appraisal and have their personal development plans and training needs identified and where possible met.

· Lead and be responsible for ensuring that they and all team members fulfil our duties regarding Safeguarding Adults.

Key Practice Issues and Responsibilities:

· Facilitate pro-active links and relationships between the Team and the wider community giving advice and guidance to fellow professionals, carers, voluntary organisations and the public.

· Foster collaborative working relationships with the whole system.

· Ensure appropriate and timely transfer of care between services.

· Ensure that all practitioners maintain accurate databases.

· Facilitate regular review of caseloads, providing guidance and support to ensure all care is of a high quality and is evidence based.

Leadership:

· The post holder will be responsible for providing senior clinical support to the duty system and will also be expected to work as frontline practitioner on duty.

· The post holder will lead on the setting, monitoring and maintaining of standards of care within the Care Home Pathway Team and will advise the Sector Manager on clinical issues which may have a negative effect on the standards of clinical practice being delivered.

· The post holder will provide clinical leadership for a range of clinician’s in their specific area of practice, providing advice on all clinical issues within that service.

· The post holder will support clinician’s with updating their relevant knowledge and skills.

· The post holder will work closely with the Sector Manager and Advanced Practitioner and provide cover in their absence. They may at times have to take on some of the operational management responsibilities should the need arise.

· Participate in the recruitment and selection of clinical staff for the service.

· Participate in the appraisal, staff performance management and other monitoring systems used within the service area.

Clinical Duties and Responsibilities:

· Responsible for a defined complex caseload within the team. Using specialist Mental Health knowledge and skills, carry out individual clinical assessment, plan and formulate a range of interventions and treatment options within care plans, implement and evaluate packages of care.

· Undertake the task of care coordinator, participate in CPA and Caseload review meetings, discharge planning meetings involving service users, their carers, other professionals, and agencies, attend inpatient reviews and act as client advocate if necessary.

· Responsible for providing advice and guidance to care homes through the established Care Home Pathway Advice and Guidance Service.

· Facilitate the development and use of care pathways by the multi-disciplinary team.

· Treat clients with short term interventions where assessment has indicated this. Assess and treat clients with complex, severe and enduring mental health problems who may be on a community section of the Mental Health Act.

· Facilitate the transfer of client from Primary Care to Mental Health Services as appropriate and discharge clients from case load to appropriate agencies when clinical interventions are completed.

· Act as a support and specialist advisor to GP’s and primary care colleagues as appropriate.

· To be part of the Senior Staff Group and attend relevant regular meetings as scheduled.

· Ensure that effective communication systems exist between the team, Primary Care and wider Integrated Care service.

· Work with team colleagues to achieve and maintain Quality Standards.

· Participate in foundation standards and service accreditation programmes.

· To attend Tribunals and other Multi-Disciplinary Meetings and prepare reports.
