Job summary

The Surrey Integrated Service for Young People [HOPE Service] operates from two bases, East Surrey [Woodside Lodge] and West Surrey [Worplesdon Road] and provides a Service for those young people aged 11 to 18 years old presenting with severe mental health, behavioral, emotional and relationship problems. The Service is the link between CAMHS Community team, Surrey Families’ Directorate [Social Care and Education] and the Specialist inpatient services. It offers support to CAMHS community team and Surrey Families’ Directorate, through structured Day Programme and Outreach Teams working in community settings. The Service also provides transitional support for those young people in psychiatric hospital in the process of returning to their local community. The Service considers referrals for psychiatric hospital and from the CAMHS Community Teams as well as Children’s Services Teams and advises of the best appropriate care; this ensures that young people get access to the right level of treatment.

Main duties of the job

Suitable for someone who has effective active listening skills (questioning and summarizing), who can use good use of empathy and diplomacy, build good rapport, knows their own limitations and has good time management.

Job responsibilities

Experience required/ Key Responsibilities

To manage a caseload of young people who are high risk

with complex mental health and social care needs.

To provide comprehensive mental health assessments focusing on risk

management to young people who are at risk of a hospital admission.

To recognise potential crisis situations and act responsively and responsibly to resolve the situation, seeking assistance if required.

To provide assertive outreach to young people and their families/carers

to monitor mental state and risk and help re-integrate into an educational

setting.

To promote shared decision making with Young People/families/carers

As appropriate, involvement in all aspects of care.

To monitor effects and side effects of medication and report appropriately to the prescribing consultant, Lead Clinician and Lead Nurse.

To ensure CPA requirements are met within specified timescales e.g care plans, risk assessments, organising of CPA meetings and record keeping.

To develop and maintain effective relationships with partnership agencies.

Develop creative packages of care for young people and their families/carers to try to prevent hospital admission or shorten length of stay when admission is necessary.

To be a resource for specialist advice and information to other colleagues, agencies and organisations of adolescents with complex mental health issues.

To provide evidence based interventions to young people and their families/carers in accordance with NICE guidelines.

To uphold the principles of the NMC code of professional conduct at all times.

General

a) All staff will have a sound awareness of Safeguarding issues and be clear about their respective roles and responsibilities to report, respond to and share information in relation to Safeguarding Adults, Safeguarding Children, Domestic Abuse and Public Protection.

b) All staff will have a knowledge of and adhere to the Data Protection Act and will not disclosure to any person or organisation, any confidential information that comes into their possession in the course of their employment.

c) To fully participate in the Trust’s performance review and personal development planning process on an annual basis.

d) Undertake training as necessary in line with the development of the post and as agreed with line manager as part of the personal development planning process.

e) To achieve and demonstrate agreed standards of personal and professional development within agreed timescales.

d) To contribute positively to the effectiveness and efficiency of the teams in which he/she works.

e) To undertake any other duties at the request of the line manager which are commensurate with the role, including project work, internal job rotation and absence cover.
