This is a unique and exciting opportunity to be part of a dedicated assessment and triage team within the Perinatal Mental Health Service. We are thrilled to offer this position to an appropriately skilled, self-motivated mental health nurse, occupational therapist or social worker.

Main duties of the job

There is great flexibility in the role and opportunity to be based at either of the team bases in Chertsey (North West team), Epsom (East Team) or Godalming (South West Team). We currently have two part-time positions and working hours are negotiable.

As an Assessment and Triage Practitioner you will work as part of a team in the clinical management of new referrals coming into the service alongside the band 7 Clinicians and supported by other senior members of the team each day. The role involves initial telephone or video screening assessment of new referrals as well as carrying out urgent assessments when clinically indicated and routine assessments, alongside developing an appropriate plan of care either via the team or signposting to more appropriate support.

As a member of the team, you will provide specialist assessments and recommend interventions to women during pregnancy, delivery and up to 2 years postnatal, supported by the MDT. The successful candidate will demonstrate strong communication skills and a motivation to develop robust links with our predominant referrers namely - GPs, Midwives, Health Visiting and Adult mental health services including Home Treatment Teams, Psychiatric Liaison Services and Mother and baby units.
