Surrey and Borders Partnership NHS Foundation Trust is working with our Integrated Care Systems to undertake a once in a generation reworking and shift in community mental health service provision for the people of Surrey and North East Hampshire. Working across health, social care and Voluntary, Community and Social Enterprise (VCSE) sector partners as well as our diverse community and service users, their carers and their families to co-produce and implement an innovative new model of personalised care set out in the "NHS Community Mental Health Framework for Adults and Older Adults".
Every community mental health service in England is changing in line with this new vision set out in the NHS Long Term Plan – and we’re in the vanguard of change as an ‘early implementer’ site.
This role involves both variety and complexity of working in a rapidly evolving programme of whole system change. We are not looking for more of the same but to continue our shift in approach to transformation and this presents a fantastic opportunity to be part of our radical journey as we look to appoint an experienced Senior Community Mental Health Practitioner.
Main duties of the job
This is a new, exciting and innovative position that will provide expert support to our Community Mental Health Recovery Services (CMHRS) as they prepare and mobilise towards our ‘One Team’ approach for fully integrating with new primary care based mental health teams, formed around local Primary Care Networks (PCNs). You will be responsible for providing short term specialist mental health advice and education to primary and secondary care colleagues, as well as providing clinical leadership, in conjunction with the Service Manager, to deliver a thorough caseload review. You’ll also use your expertise to implement strategies in both primary care and mental health that promotes an environment of user involvement and recovery.
We are looking for autonomous, driven, solution-focused and confident practitioner(s). This role provides the opportunity for continuing professional development (CPD); with scope to influence service innovation and development with our colleagues across primary care, social care, VCSE sector and people with lived experience. This role is for practitioner(s) who pride themselves on delivering a high-quality holistic approach for patients needing mental health support.
