Job summary

My job makes better lives by leading and managing young people who are 16 or 17 years of age, who are highly complex in presentation and, working in partnership with a Multi-disciplinary team consisting of representatives from police, ambulance, A/E and Mental health service alongside the young person, collaboratively making a difference to their lives.

Job Overview As part of a partnership Multi-disciplinary team, for example police, to ensure the service operates in a safe, effective and efficient way as part of a whole system. Foster a culture that encourages and enables individuals to have an active role in identifying, planning and meeting their individual needs. Support strategic leadership in relation to service improvement and quality evaluation

Main duties of the job

To exercise good personal time management, punctuality, and consistent reliable attendance

To take a lead on operational planning, implementation of policy and service development within the team

To actively lead on service developments including reviewing the evaluation of this pilot for future provision

To broaden research and development skills through participation in the planning, evaluation and audit/research of practice, clinical pathways, and protocols

To take the lead in the planning and delivery of the service business and clinical governance plans and delivery of the Trust business and clinical governance plans

To demonstrate a high level of understanding and application of national guidelines and health legislation (e.g. National Clinical Guidelines, NSFs)

To promote a positive image of the Trust

To develop and maintain working relationships with all other professionals across the pathway to facilitate high quality seamless care and whole system service development

Job description

Job responsibilities

· To be able to manage a caseload of highly complex people who are 16 or 17 year old

· To be able to carry out appropriate assessments for the designated caseload

· Addressing, for people who use this service with multiple and complex needs, using advanced clinical reasoning skills and knowledge of evidence based practice

· To work with people using our services in identifying and agreeing treatment goals based on assessment outcomes, which balance the complex interaction of risk, safety, client choice, independence, areas of conflict and eligibility of services

· To use communication, reasoning, and negotiation skills to establish a therapeutic relationship in highly complex cases

· To plan and implement individual plans, addressing all areas of risk associated with an individual’s ability to function independently

· To monitor, evaluate and modify interventions in order to measure young people’s progress and treatment effectiveness

· To represent the team and participate in relevant team/network meetings, clinics, and ward rounds to contribute to the discussion and planning of the care of people using services. To use these forums to seek opinion from the team to inform own caseload analysis and reasoning as appropriate

To liaise with families/carers and statutory and voluntary agencies to contribute to the discussion and planning of care and seeking opinion from others to inform own caseload analysis and reasoning as appropriate

To ensure that all documentation and activity recording is maintained according to Trust

To demonstrate the ability to reflect on ethical issues and provide guidance to junior staff as necessary

To respect the individuality, values, cultural and religious diversity of young people and contribute to provision of a service sensitive to these needs

To demonstrate a sound understanding of clinical risk, report untoward incidents, both verbally and in writing. To support and guide other team members in the correct process.

  • Lead on the setting, monitoring, and maintaining of standards of care and advise on clinical issues which may have a negative effect on the standards of clinical practice being delivered
  • Responsible for your own practice in accordance with your professional body and you will take every reasonable opportunity to sustain and develop your professional competence
  • Be a mentor and adviser for students and will work closely with the Clinical Placement Facilitator to ensure that the learning objectives are consistent with the requirements of their educational programs
  • Provide clinical leadership for a range of clinicians in their specific area of practice, providing advice on all clinical issues within that service; support clinician’s with updating their relevant knowledge and skills
  • Work closely with the Hope Service and IIT and the adults CMHRS when required
