Job summary
Due to
exciting changes, we are seeking Band 6 to Band 7 Crisis Intervention Nurses within the Mindworks Crisis Intervention Service for Surrey.
Are you an RMN, Social Worker or Occupational Therapist with experience
working with Children and Young people, looking to lead & support assessments and interventions for young people who
are in crisis?
Our team
provide a 7 day week crisis intervention service, offering discovery
conversations to children, young people & their families
presenting with emotional and mental health crisis in local acute hospitals and community bases. Our operational hours are
9am 5pm Monday Sunday including Bank Holidays. There will be one late shift until 8pm, every 2 weeks. Unsocial and weekend shifts will be expected.
The
successful candidate will hold an appropriate professional registration with
NMC or HCPC. Experience and competence in working with vulnerable people in
crisis is essential. Experience of working with children and young people is
required.

Main duties of the job
The main duties within this role are leading and supporting assessment, risk and care planning alongside Children and Young People (CYPS), their family or carers and the acute staff to safely facilitate discharge with follow up care planned in the community.
Supporting junior staff to provide high quality care, sharing learning and developing skills through supervision and training while maintaining KPI's and collecting data to support the service growth and development are all part of this exciting role. Band 6 Key Responsibilities
The post holder will work
closely within the Mindworks alliance during office hours, responding to
emergency referrals of young people presenting in crisis
The post holder will work as an
autonomous practitioner, undertaking clinical assessment of children and young
people who may have a complex co-morbid mental health needs and present in
crisis, working alongside medical, nursing and other professional colleagues to
ensure that the most effective care and treatment is provided resulting in
safe, timely discharge
To co-ordinate and liaise with
Paediatric ward and paediatric liaison nurses when children and young people
with mental health issues are admitted.
Work closely with the Emergency
Departments, the Adult Psychiatric Liaison Service, Mental Health Crisis Teams,
On Call Duty Psychiatrist, On Call Managers, Paediatric Liaison Mental Health
Nurses, ensuring young people are seen in a timely manner and risk is assessed
and a management plan is developed.
To work within a highly
experienced multi-disciplinary team involved in the assessment and treatment of
children and young people with serious and complex mental health difficulties
in crisis.
To manage own caseload from
referral to discharge/transfer, holding medium to high risk cases and ensure
timely and accurate documentation is completed.
Ensure that outcome measures
for Crisis Intervention are implemented and audited. Contribute to relevant
research and ensure that interventions on the pathway are evidence based.
Be responsible for the initial
assessment and subsequent facilitation of referral to Hope and/or discharge
with a signed care plan regarding safety and recorded on SystmOne.
Band 7 Key Responsibilities (in addition to Band 6)
The post
holder will work closely with the Mindworks alliance during office hours, responding
to emergency referrals of young people presenting in crisis:
Report to and advise referrer
on assessment and treatment outcomes and respond to requests for and provide
formal reports for other agencies.
Represent the Crisis
Intervention Service at professional/network meetings to offer
advice/consultation on complex cases who are in crisis.
Develop and implement systems
to monitor and evaluate the service
Supervise other Crisis
Intervention professionals of a lower grade and offer support and supervision
to junior team members
To undertake relevant CPD to
improve assessment and treatment skills regarding crisis intervention.
Be competent in conducting
crisis assessments.
