We are looking for innovative and enthusiastic Mental Health Practitioners to join and complement our existing experienced Mental Health teams.
You will be a core member of the Multi-Disciplinary Team, participating in the provision of on-going assessments, care planning, treatment and support of people with serious and enduring mental health problems and people in crisis to promote health, independence and recovery.
You will have:
Current full UK NMC registration
Band 5: One years experience working with people with mental health issues (can be pre-qualification)
Band 6: Two years post qualification working in the community with people with severe and enduring mental health illness

Main duties of the job
You will be a core member of the multi-disciplinary team, participating in the provision of on-going assessments, care planning, treatment and support of people experiencing functional or organic mental health issues.
Care-coordination for those on caseload subject to CPA.
Member of team Duty rota
Engagement with carers and families to ensure Triangle of Care maintained.
Supervision and line management of junior colleagues (dependent on role).

Community Mental Health Nurse - Band 5/6
Teams based in Aldershot, Chertsey, Epsom, Frimley, Guildford, Redhill.
My job makes better lives by ensuring that the Community Mental Health Service has efficient and appropriate clinical leadership and support to enable the service to have good outcomes for people who use it.
Suitable for someone who is looking for a busy, challenging and complex environment in which to develop and establish effective working relationships with a wide range of internal and external stakeholders, and supporting the implementation and delivery of high quality Single Point of Access for Mental Health Referrals. Flexibility in meeting the needs of the service is required, especially in view of supporting expanding and developing the service
The post holder will have excellent assessment skills and the ability to plan, implement and evaluate care in a multi-disciplinary team setting. You will be up to date with training and professional development, and have the necessary skills and qualifications, or be willing to attain them, to mentor students within the team.
The Post Holder will act as care coordinator, where appropriate, to patients who require the skills of a Mental Health practitioner, and provide a holistic, clinical, autonomous, mental health service to enable individuals to engage in the best standards of care available.
QUALIFICATIONS REQUIRED:
· RMN/BSC Hons or Diploma in mental health nursing
· 1/2 years post qualification experience (dependent on band)
· Membership of a professional body (NMC) registration
EXPERIENCE REQUIRED:
- Experience in Older People’s community mental health nursing (preferable and dependent on band)
- Experience in care coordination and caseload management
- Basic IT Skills including the ability to use a computer
- Must be able to drive for work purposes
MAIN DUTIES AND RESPONSIBILITES :
- To deliver a high quality community mental health service to Older People with mental health problems, in a multi – professional framework.
- To prescribe services that meet the user / carer needs, in conjunction with others.
- To act as key worker / care co-ordinator for named individuals, within the Care Programme Approach process, including Single Assessment.
- The ability to work alone.
- To provide high standards of care, which are delivered in a professional manner.
- To ensure that the care delivered is based on best practice, and is regularly reviewed.
- To attend and ensure all referrals are passed through CMHT allocation meeting, and assessed for acceptance to the specialist team.
- To ensure all records and reports are kept up-to-date, and up to the standard expected of this grade of nurse, in accordance with the Caldicott Report.
- Keep an accurate, up-to-date diary and record of visits.
- Be responsible for the continuous overall management of a defined caseload.
- To receive and give clinical and management supervision / guidance.
- To supervise juniors / other staff in the delivery of care and treatment to individuals and groups.
- To support junior staff to manage caseloads effectively.
- To act as a positive role model for junior staff, assisting / guiding them in their duties.
- To provide information to the Line Manager, any incidents that may affect the standing or running of the service.
- To develop close working relationships with voluntary / statutory services, in particular Social Services and members of the Primary Care Team, and participate in the system of integrated team working.

CLINICAL ROLE :
- To provide a holistic, autonomous and clinical community nursing service by attending to referrals and the ability to work alone.
- To assess clients according to agreed care pathways, and with appropriate assessment tools.
- To refer to other appropriate services, according to assessment of need.
- To participate in the Team’s Duty Rota.
- To treat clients with short term interventions, where assessment has indicated this.
- Ensure audit and evaluation take place as expected and required.
- Attend relevant clinical / business meetings.
- To act as support and specialist advisor to the GP, and primary care colleagues.
- Provide supervision and appraisal for junior staff.
- Carry out agreed nursing procedures and clinical treatments in accordance with agreed procedures.
- To use the Trust Clinical record system and other IT systems as appropriate.
- To attend and prepare reports, for MDT meetings.
- To ensure the safe storage and recording of medicines within the clinical area.
- To administer intra muscular injections, as prescribed according to Trust and National guidelines and policies.
