The NW Community Mental Health Team for Older People (Woking Locality) is seeking an enthusiastic Band 5 Community Psychiatric Nurse to join the team.
The successful applicant will be part of the multi-disciplinary team delivering comprehensive assessments and a range of approaches, interventions and treatments to older people experiencing severe and enduring mental health problems.
Main duties of the job
You will manage a caseload of clients, and use evidence based/client-centred principles to assess, plan, implement and evaluate interventions as part of the holistic approach. You will need to demonstrate excellent communication skills and be able to work collaboratively with a multidisciplinary team. You will be committed to the delivery of a high-quality service, working both independently and as part of a well-established, innovative team in meeting the needs of people who use the service and their carers.

A successful candidate would be employed at Band 5 and, after completing a preceptorship and/or a period of supervised practice and demonstrating evidence of competence, would then be supported to move to a Band 6 role.
The candidate must have a full and valid driving licence and access to a vehicle for work purposes due to the community element of this role.
