Job summary

We have an exciting opportunity to recruit a new Mental Health Practitioner in Aldershot PCN through Additional Roles Reimbursement Scheme (ARRS).

We are seeking to find Mental Health Practitioners who can come and make a difference. Providing high quality interventions in our new primary care integrated mental health service, you will be an important member of our primary care team in the Primary Care Network (PCN - which is a partnership of a few GP Practices).

The introduction of these roles is part of a wider transformation and expansion of community-based mental health services set out in the NHS Long Term Plan. The practitioner can access and draw down on secondary mental health services and help provide a bridge between primary care and secondary mental health providers.

The current Vacancy is a Full-Time (37.5 Hrs, 9am-5pm, Mon-Fri, working from the GP Practice/s), Permanent role at Band 6 in Aldershot PCN.

The post-holder in this Vacancy will be required to work from 3 GP Surgeries within this PCN while a single GP Surgery address is given on the advert as an example. Please refer to the supporting document attached with this vacancy for details of all the Job Locations.

Main duties of the job

The practitioner will be embedded as part of the Primary Care Network Multi-Disciplinary Team. Working alongside the other roles, they will also be part of the community mental health Multi-Disciplinary Team (MDT) with ready access to a wide range of support and expertise across both MDTs. It is envisaged that the focus of a practitioners work will generally be people whose needs cannot be met by IAPT services but who may not need ongoing care from secondary care community mental health services. The practitioner can facilitate onward access to other secondary care-based services, or third sector services integrated within the mental health pathway.

Successful candidates must be experienced mental health practitioners and have experience of working in a Primary Care and/or Community Mental Health Team setting. You will be a confident communicator with colleagues and stakeholders alike. Above all you will have the ambition and drive needed to work within an environment of change and innovation.

The candidate must have a full and valid driving licence and access to a vehicle for work purposes due to the community element of this role.

Job Overview

Surrey Heartlands is an ‘early implementer’ site for community mental health transformation. The adoption of ARRS Mental Health Practitioners (MHP) is an exciting and new development that will build on and complement the progress of new models made to date (i.e. GP integrated mental health services - GPimhs) and represents an expansion and opportunity for better joint working across primary care and adult secondary care mental health systems.

Suitable for someone who is proactive and passionate about innovation and providing a high-quality holistic approach to the care of people who have been identified as in need of mental health support in a primary care setting.

Key Responsibilities

  • To review patients presenting with mental health issues, consider a range of options regarding treatment interventions providing them when appropriate and liaising with GPs and the GPimhs team.
  • To act as the first point of contact for GP’s, to offer advice, support and sign-posting to appropriate agencies.
  • To receive and communicate sensitive information to clients in situations which may be highly emotive, and where barriers to acceptance and understanding may need to be overcome, requiring developed communication and empathic skills.
  • To carry, manage and be responsible for own workload and the quality of care they deliver within the clinical area that he or she is responsible for, liaising regularly with GPs, practice staff GPimhs/MHICs team and other relevant organisations, and undertake administrative tasks related to caseload where necessary.
  • To maintain and update patient records on the computer in accordance with practice policies and professional guidelines e.g. NICE and other evidence-based guidelines.
  • To liaise with appropriate resources when necessary (e.g. Welfare Rights, Housing Agencies, Voluntary Agencies, Day Services etc.).
  • To ensure effective lines of communication are maintained with GPimhs to promote good working practices and ensure that service user information is passed on appropriately to other services where required.
  • To participate in professional meetings as necessary where there is notification of serious concern i.e. Safeguarding and risk management concerns or in multi-disciplinary reviews.

Subject to reasonable adjustments in respect of any disability, the post holder must possess a valid driving license and be able to drive and have the unrestricted use of a car or motorcycle for work at all times.
