Job summary

As a member of the senior team, the role is an exciting opportunity to provide clinical leadership, supervision and guidance to nurses within the Reigate Community Mental Health Recovery Service (CMHRS).

The role includes providing clinical supervision to all nursing staff; being a mentor and adviser for student nurses and working closely with the Clinical Placement Facilitator to ensure that the learning objectives are consistent with the requirements of the educational programmes.

The post holder will lead setting, monitoring and maintaining standards of care and will advise the service manager accordingly

The post holder will facilitate clinical sessions, assess / triage people according to agreed assessment tools, offering short- term interventions based on psychological therapies and offer professional advice to people and their carers, other professional, statutory and voluntary agencies

The post holder will deputise in the absence of the Service Manager and will be responsible for delegating duties to other members of the team, taking a lead on the day to day running of the team, and undertaking management responsibilities as requested.

Facilitate the development and use of care pathways by the multi-disciplinary team

To be part of the Senior Staff Group and attend relevant regular meetings as scheduled

To supervise and participate in the safe carriage, storage and delivery of medication to clients both in the clinical area and domestic environment

Act as a health educator for people, relatives, carers and staff. Advise on issues of physical and mental health so as to engender a positive approach to health and strengthen the person’s capacity to manage her/his illness, symptoms and crises

Be responsible for creating an environment which is conducive to learning and development

Participate in the recruitment and selection of staff for the service. Participate in the appraisal, staff performance management and other monitoring systems used within the service area

Comply with and adhere to the Professional Code of Conduct as outlined by the Nursing & Midwifery Council and other appropriate professional practice guidelines

Be accountable for her/his practice and take every reasonable opportunity to sustain and improve their knowledge and professional competence and that of the Team

Ensure that nursing practice is based on current knowledge and is evidence based

Have due regard to the workload of and the pressure on colleagues, and take appropriate action

Job description

Job responsibilities

The post holder will provide a holistic, autonomous nursing care and maintain communication with primary care professionals and other professionals and agencies through regular informal and formal meetings. These meetings will include a review of patients within the practice/service with mental health problems.

The post holder will facilitate clinical sessions and assess / triage people according to agreed assessment tools, offering short- term interventions based on psychological therapies and offer professional advice to client / patients and their carers, other professional, statutory and voluntary agencies. They will facilitate the transfer of patients between primary and secondary care and between the different specialist teams within the Trust and to care coordinate clients on both standard and enhanced CPA.

The post holder will lead on the setting, monitoring and maintaining standards of care and will advise the service manager on issues which endanger standards of practice within their specific area of practice.

The post holder will be responsible for their own nursing practice in accordance with Nursing and Midwifery Council Guidelines and will take every reasonable opportunity to sustain their professional competence.

Dimensions: To hold a continuous responsibility for a defined caseload in the mental health team. Liaise between primary care and primary care mental health teams to ensure clear communication and seamless care pathways, thus enabling access to the appropriate level of care and intervention.

Key Relationships: The post-holder will work collaboratively with other members of the multidisciplinary team and in partnership with colleagues from other Mental Health Services, other care groups and Surrey County Council.
