Job summary

We are looking for an enthusiastic, flexible individual with excellent communication skills to join our team of nurses.

The priority will be to provide excellent discharge planning for the patients on Swift ward.

The role will involve working with mainly elderly patients and their families who often have complex discharge needs.

You will work closely with the Nurse in Charge and the discharge team to ensure safe and effective discharge processes are in place, and support the Ward Manager in the day to day discharge management of patients.

Main duties of the job

The Key role of the job is to plan and coordinate discharges.

The successful candidate will need to demonstrate excellent communication skills and show the ability to sometimes have difficult conversations with patients and their families. IT systems will require constant updates to ensure all members of the team are aware of current discharge plans.

The successful person will need to be able to work well in a team but also be able to work independently at times.

The post is ward based but good working relationships need to be made with the discharge team to ensure best outcomes for our patients.
