16 month contract

We have an exciting opportunity to recruit to a Breast Cancer Support Worker, to support patients on the breast cancer pathway. This role will support the Breast Clinical Nurse Specialists in providing information and support to cancer patients and their families. It involves triaging calls and escalating concerns to facilitate cancer patients through cancer pathways and treatment programmes. This role is pivotal in the delivery of holistic needs assessment and care plan and sign posting to local support services.This role will also support the delivery of open access follow up for breast cancer patients which includes support and escalation of patients with disease recurrence through the single point of contact and involvement in developing cancer wellbeing programmes. The role requires a knowledge or willingness to work with hospital systems, cancer care and a multidisciplinary team.Excellent communication skills are essential in providing support to individuals affected by cancer and their families including the ability to manage level 1 psychological distress and recognise and escalate level 2 psychological distress. Ability to demonstrate a compassionate approach and to provide empathy with patients who may be experiencing a difficult time in their life is essential.A commitment to further improving services for cancer patients and their families is essential.

Main duties of the job

The Cancer Support worker will:

• Work with the breast clinical nurse specialists to support patients undergoing a breast cancer diagnostic pathway, patients with a cancer diagnosis or patients living with and beyond cancer.

• Work closely with clinicians, specialist nurses, admin staff and all members of the Multi-Disciplinary Team to ensure that patients are actively managed through the cancer pathway.

• Support the delivery of personalised care following a cancer diagnosis

• Support and escalate recurrence of cancer to the nursing team.

• Support ongoing wellbeing programmes

• Support risk stratification and delivery of open access patient follow-up for breast cancer patients

• To work with and cross cover single point of contact and re-entry contact number

• Coordinate the follow up investigations required post cancer diagnosis or treatment and provide a point of contact for patients post cancer treatment

• Support patients and carers to access appropriate cancer information and support

• Ability to promote patient empowerment and self-management strategies

Information and reporting

• To coordinate and organise appointments and information.

• Communicate and support the delivery of Macmillan information to patients and carers.

• To maintain up to date knowledge of all targets and processes relating to the patient pathway process.

Technology or system support

• To provide effective support to ensure smooth facilitation of treatment and care

• Maintain an accurate and validated data base

• Support remote surveillance projects

• Support and coordinate the EHNA tracking, ensuring the monitoring patient needs

• Provide fast and accurate clinical correspondence administration service with proficient use of appropriate software and systems to meet the required Trust’s standard of work.

Service improvement

• Provide a point of contact for patients following treatment

• Support the delivery of the Recovery Package in line with the Cancer Alliance

• To work under the guidance and supervision of clinical team

• To identify delays in the patient pathway and to follow the trust escalation policy to raise awareness to the Team Lead for Macmillan and Lead Cancer Clinician as appropriate.

Communications and Engagement

• Face to face and telephone communication and engagement with cancer patients and families

• Ability to deal with level 1 distress and recognise and escalate concerns regarding psychological wellbeing and management

• Provide telephone advice and navigate to sources of support (third sector –including Macmillan)

• Responsible for dealing with enquiries from patients, (written and via telephone/email)
