Job summary

Would you like to work in a friendly multidisciplinary team providing home-based early rehabilitation and support to patients following stroke?

We are looking for a Rehabilitation Assistant to join the Early Supported Discharge (ESD) team for Stroke in South West Surrey. Our team includes Physiotherapists, Occupational Therapists, Speech and Language Therapists, Specialist Stroke Nurses, Psychologists, Rehabilitation Assistants and Administrator support. Youd be working directly with patients, in their own homes, to assist with rehabilitation programmes and deliver some aspects of patient education. Some intervention is conducted by video or telephone. Youd also support some non-clinical and administrative duties.

You need a flexible approach, plus good organisational and prioritisation skills.

We have another base in NW Surrey, but staff are managed as one service for ESD. All staff are expected to work flexibly across our sites according to service need.

Community experience is an advantage, as is working with people with stroke or other neurological or complex conditions. Considerable amounts of driving are required - you must have a valid UK license and access to a car.

We support work/life balance and are happy to discuss flexible working at interview. Assistants may be required to work weekend shifts on a rota basis, as part of 7 day service provision.

Main duties of the job

  • To be part of the multidisciplinary team (MDT) and implement specific treatment programmes or interventions delegated to them by qualified therapy staff
  • To manage own clinical caseload
  • To contribute to patient education
  • To work in close liaison with the team clinicians (Physiotherapists, Occupational Therapists, Speech and Language Therapists, Specialist Stroke Nurse/Clinical Nurse Specialist, Psychologists) to assist them with their duties
  • To carry out rehabilitation as determined by patients needs
  • The post holder will be dealing with a varied caseload of patients
  • To undertake non-clinical duties related to the treatment and management of patients - photocopying, filing, ordering/delivering equipment, recording patient contacts on Trust IT system / medical records, arranging follow up appointments etc.

To work in collaboration with other team members to provide a rehabilitation service for patients within the speciality, assisting in the implementation of programmes to meet individual needs

To provide therapeutic intervention for individual patients to maintain or improve function in mobility, cognition and communication, as well as personal / domestic activities of daily living and social activities

To issue mobility equipment / aids in the home following prescription by an ESD clinician

To carry out treatment programmes, adapting and progressing the rehabilitation of patients within the guidelines/scope set by the prescribing therapists. This will include modifying treatments to take account of changes in the patients physical and mental state, and other factors within the community such as weather conditions

To work with patients individually as directed by qualified therapists/nurses

To work in conjunction with patients and carers on specific issues, as directed by the MDT, and to deal with their concerns as they arise

To be aware of problems that could affect the patients progress and report back any changes to the clinicians

To maintain accurate documentation for every patient contact

To be aware of risks involved as part of delivering clinical care and manage this within own patient caseload, taking advice from /reporting back to senior clinician

To demonstrate physical ability to carry out assessments and interventions including manual and therapeutic handling techniques. This post involves frequent light to moderate physical effort

Communicate any changes in care plan or treatment approach to MDT colleagues as necessary

Report back directly to the MDT, particularly any changes in functional abilities, relevant information gained or patients comments linked to treatment

Maintain accurate and legible records of all interventions and management, using agreed format, in accordance with standards set by the therapy service
