Job summary

Cancer Services are looking for a highly motivated and enthusiastic individual to join them in supporting the Trust in achieving and sustaining the cancer targets. Excellent organisational skills will also be required to ensure that patients are tracked, and their progress monitored along their pathway using hospital computerised data and tracking systems.

Main duties of the job

To work closely with members of the clinical and non-clinical teams across the Trust in developing policies and procedures to ensure seamless management of patients across all departments. To be responsible for ensuring the accurate and complete collection and management of the national cancer minimum dataset for all patients discussed at MDT, and to ensure all patient pathways are correctly mapped and appropriately tracked. To actively monitor and manage the pathway of suspected and diagnosed cancer patients in defined specialties referred to the Trust, to ensure that the patient reaches the point of treatment within relevant time-scales. To act as the patient navigator, and follow the patient journey from referral, through diagnosis, to treatment. To provide a point of contact for patients to liaise and discuss concerns, resolve and support through good communication and administration. To liaise with members of the individual specialties MDT, and across the Trust as a whole, to ensure coordination of all elements of the patient pathway. This may include highlighting patient target dates etc. to ensure that investigations are carried out and results received in a timely manner.
