Job summary
Cancer support workers play a key role in supporting cancer patients throughout their cancertreatment and living with and beyond cancer. Working within the multidisciplinary team, thepost-holder will ensure patients receive the care, information, and emotional support theyneed. This role will focus on assisting with practical administrative tasks, care coordination, and helping patients navigate the complexities of the healthcare system. A confident understanding to the impact of cancer on individuals and families is needed as the role requires direct contact daily via phone or face to face during a worrying and difficult time for this group of people.The post-holder will work closely with clinical teams to ensure seamless patient care, focusing on person-centred, compassionate support while adhering to relevant policies and guidelines. Possessing confident IT skills with ability to interact effectively with health professionals to benefit collaboration and excellent service provision is vital.We are unable to support sponsorship for this role.
Main duties of the job
Coordinate care by providing a seamless pathway for all cancer patients throughout theirtreatment journey and follow-ups. Often being the first point of contact, the cancer supportworker will triage calls and offer advice, and support as able, signposting and referring on asrequired.Supporting Cancer Nurse Specialists by taking on administrative roles related to supportingHolistic Needs Assessment, document, tracking and recording all patient and service userinteractions.Providing holistic support and information and signposting to existing support in the hospital and community. Working alongside registered practitioners to improve care for people with cancer. Based in the cancer services department there is across site working to outpatient clinics, inpatient areas, and administrative offices. Full training and ongoing supervision will be provided.
