An exceptional prospect awaits within the Adult Diabetes Specialist Nursing Service at Ashford and St Peters Hospitals NHS Foundation Trust. We extend an invitation to experienced adult diabetes specialist nurses to take on the role of Senior Adult Diabetes Specialist Nurse. This is an opportunity to align your expertise with our commitment to exceptional patient care.

In this capacity, you will engage closely with a team of experienced diabetes specialist nurses, alongside Diabetes/Endocrinology consultants and various members of our multidisciplinary team. Your contributions will span both inpatient and outpatient care. This role needs service delivery across our Trust's two sites and may require for you to contribute in running a 7-day service.

We are in pursuit of an enthusiastic and dynamic professional, with significant experience as 7 adult diabetes specialist nurse. As an ideal candidate, your experience should encompass patient care involving insulin pumps, ideally supported by nurse-prescribing credentials. Main duties of the job

The post holder will:1. Provide and manage expert specialist clinical practice to out-patient clinics and in-patients for patients with diabetes.2. To lead in the development of evidence-based clinical nursing practice in line with current research and guidelines.3. To ensure that patients and their families are fully informed and supported in the management and care of their diabetes.4. To link with the wider healthcare teams in order to increase knowledge and delivery of high quality and appropriate diabetes care in both inpatient and outpatient clinical settings.5. This post holder will be part of the delivery of structured education and support of patients on insulin pumps.

  • The post holder will work as a member of a team made up of Adult Diabetes Specialist Nurses, dietitians, consultants and other members of the multidisciplinary team.
  • Work in partnership with the other diabetes specialist nurse and will act as an expert resource to health care professionals, family and carers.
  • Develop and participate in Trust diabetes initiatives.
  • Be responsible for maintaining a professional profile at all times.
  • Act in accordance with local policies and procedures
  • Conform to the regulations laid down by the Data Protection Act and also respect confidentiality of all staff and patient information.
  • Ensure the health and safety of staff, patients and visitors, recording and reporting actual or potential problems promptly as outlined in Trust policies.
  • Identify actual or potential clinical risks and take appropriate action, using when required the appropriate reporting structures
  • Demonstrate sound IT skills and be prepared to undergo training to use electronic patient record system e.g. Cerner

Clinical

  • Maintain effective communication with patients, carers and professionals to ensure seamless service delivery, and to act as a first line contact resource, using highly developed specialised knowledge underpinned with experience and theoretical knowledge for diabetes related issues.
  • Be able to impart highly complex information, which requires developed communication and reassurance skills in order to empower people with diabetes to self-care, such as starting people on insulin therapy. Having the ability to deal with distressing circumstances associated with diabetes, e.g loss of vision, amputation, heart attacks etc.
  • To participate in Diabetes Clinics, formulating individualised patient treatment care programmes in conjunction with the patient, multidisciplinary team and initiating changes in management of care.
  • To be responsible for assessing patients diabetes needs, identifying physical, psychological, physiological, emotional, spiritual and cultural needs in relation to their condition and proposed treatment.
  • To act as a clinical expert in advising patients who present with diabetes related problems and provide clinical advice and support to ward-based staff and junior members of the team.
  • To act as the patients advocate when relevant and educate on all the treatment options to enable a patient to make an informed choice, negotiating with the medical team to agree a care pathway.
  • To ensure patients, carers and health care professionals are given verbal and written information regarding planned care.
  • To identify vulnerable groups and provide support, care and education to enable them to be self-caring in the management of their condition.
  • Receive/triage patient referrals received through electronic patient record system (Cerner), phone and email referrals according to agreed protocol.
  • To work across organisational boundaries and in conjunction with the multidisciplinary teams.
  • Under agreed guidelines, protocols and using non-medical prescribing undertake the initiation and stabilisation of insulin and oral hypoglycaemics agents.
