We are excited to advertise 1 WTE Diabetes Specialist Nurse post in Ashford and St Peters diabetes team.We are a friendly, enthusiastic multi-disciplinary team comprising:
7 consultants
6.6 WTE Diabetes Specialist Nurse team (plus 3 new DSN posts advertised)
1.6 WTE Diabetes Specialist Dietitians
Diabetes Specialist Podiatry team supporting diabetes foot service
Diabetes & Endocrine Specialist Registrar
Junior doctor ward team
We are fortunate to have the Stephanie Marks diabetes centre on St Peters Hospital site in Chertsey, built with £1million fundraised by the local community led by Ian Botham.
Diabetes centre provides a hub for delivery of care for diabetes paediatric, transition, adult clinics as well as diabetes community podiatry.
Adult diabetes service provision:
Inpatient diabetes care
Type 1 diabetes care includes QISMET accredited STEPH Type 1 diabetes course, CGM and closed loop insulin pumps
Diabetes young adult service (age 19-25)
Diabetes antenatal service
Diabetes foot service
Diabetes renal service
Complex Type 2 diabetes including people age under 40 years old.
This is a development role to provide specialist nursing for the care of the patient with diabetes, their families and other health care professionals.
This post will enable the applicant to develop their knowledge of diabetes and its management. The applicant will be supported to provide evidenced based care to ensure clinical outcomes that meet the challenging targets for the service.
Main duties of the job
In conjunction with other members of the Diabetes Team, to provide specialist diabetes care to adults and young people who attend the Diabetes Clinics.
To assess, plan, implement and evaluate holistic specialist nursing care in both the hospital and outpatient setting and maintain appropriate records for an individual caseload.
Act as a source of specialist advice on management of diabetes, including the provision of basic dietary advice and the management of acute complications - either face to face or by telephone on a day to day basis.
To provide individual specialist teaching to patients and carers to facilitate the development of self-care skills and knowledge, at the time of diagnosis.
To facilitate discharge from hospital for newly diagnosed or pre-existing Diabetes patients, arranging follow-up in clinic or by telephone as need dictates.
To establish and maintain effective communication with patients with diabetes in order to facilitate appropriate responses / adaptation to life with a long term condition.
Adjust insulin doses in liaison with the medical team and the senior clinical nurse specialist.
To minimise hospital admission by providing advice/information and psychological support by Telephones, particularly during periods of inter current illness.
