Job summary
Provide a comprehensive administration service to the Complex Discharge Team and participate in the IDB/ASC administration team.
To liaise with the CSH community co-ordinator daily and identify, refer and co-ordinate the transfer of rehabilitation and stepdown patients to the community.
- Carry out daily administrative duties required within the Complex Discharge team.
- Populating the MFFD Tracker for the daily MFFD Call and attend call and update IP PTL.
- Accurate record keeping of outcomes, referrals and discharge information on Cerner, and IP PTL.
- Attend CSH calls to discuss all Rehab referrals twice weekly.
- Participating in the answering of IDB phone calls and referring onto the appropriate person.
- Ordering of stationery, uniforms and items for the Complex Discharge Team.
- Sending of reports, audits and data as required by the Complex Discharge Team.
- Self-Manage workload and priorities.
