Job responsibilities

1. Assessment of Care:

Observe and record the patients condition and needs and report changes in their condition.

In any clinic before and after consultation, liaising closely with Registered Practitioners for advice/support/action;

Be able to assess medical records pre consultation to assess patients expected conditions and needs;

Report changes relating to any specific department competencies performed to a Registered Practitioner;

Record and document information in a professional manner in patient/department records.

1. Planning of Care:

Contribute to the formulation of patient care under the close supervision of a Registered Practitioner.

Be aware of others roles and boundaries;

Be able to check medical records, being aware of actioning specifics relating to individual clinics, preparing for delivery of patient care;

Good knowledge of set up of clinic/equipment.

Refer to care plans when delivering care as appropriate;

Inform the Registered Practitioner of any concerns regarding patients social care condition, support care and needs, or whenever there are changes to the patients condition which affect effective discharge.

1. Delivery of Care:

Liaise with a Registered Practitioner in the delivery of agreed components of patient care.

Act on the delivery of care under the close supervision of a Registered Practitioner relating to any departmental skill competencies achieved.

1. Evaluation of Care:

Contribute to the evaluation and re-assessment of patient care under the close supervision of a Registered Practitioner.

During the patients episode within Outpatients, alert and inform a Registered Practitioner of any needs, using departmental and competency related knowledge to question, including a Registered Practitioner in any decision making.

1. Clinical Skills:

Undertake specific clinical activities, relevant to the clinical area, that demonstrate advanced technical and/or caring skills and that require additional training and assessment.

Work towards achieving departmental skill competencies relating to individual clinical areas worked (see attached list);

Chaperone patients during examination/consultation

Report to a Registered Practitioner any adverse incidents.

1. Development of Self:

Achieve an accredited, vocationally relevant qualification.

Following discussion with Department Manager.

Acquire, maintain and improve the knowledge and skills required to perform all agreed health care activities competently. This involves undertaking in-house training as agreed with the Department Manager.

1. Development of Other Health Care Staff:

Contribute to the development of others, under the close supervision of a Registered Practitioner.

Assist the Registered Practitioner with co-mentoring new HCAs;

Contribute to the development of junior staff to assist with developing departmental competencies.

1. Department Responsibility:

Share responsibility for co-coordinating a key aspect of service provision.

To individually take on a designated/specific responsibility/ies within the department, aiming to assist in efficient departmental management;

To actively assist the Registered Practitioner in taking forward any departmental initiatives in a positive, forward thinking, developmental manner;

Adhere to and support all inter-departmental initiatives and protocols/guidelines; including standby arrangements for weekends and bank holidays.

These responsibilities may be amended to reflect departmental needs and patient care needs.

1. Principles of Conduct:

Work in accordance with the principles identified in the Oxford Radcliffe Hospitals NHS Trust Code of Conduct for Health Care Assistants (see Appendix 2).

Essential

• Good command of both written and spoken English
• Care Certificate
