Job summary

An exciting opportunity has arisen to join our Cardiology Outreach / Liaison ANP team at the OUH, to join a service provided by our team of highly valued Cardiology Liaison Advanced Nurse Practitioners. The successful candidate will be required to work across our two acute hospital sites, the John Radcliffe Hospital in Oxford, and the Horton General Hospital in Banbury to provide Cardiology outreach cover 7 days a week as part of a team

We are looking for an expert nurse with a strong interest in Cardiology who is a team player, and who has the ability to lead and develop practice in order to provide a high quality service throughout the patient pathway for patients and their families, working with clinical colleagues to provide information, advice and support to patients with cardiac conditions.

Main duties of the job

The Post holder will work as part of the Cardiology team to provide advanced clinical assessment, support and onward referral for patients attending the Emergency Department, Emergency Assessment Unit, and any department or ward in the OUH sites and undertake to assess patients presenting in the Rapid Access Chest Pain clinic

The aim of the post holder is to expedite assessment, including appropriate tests and investigation based on clinical need and to provide quality of care and support for patients and their families.

The Post holder will liaise across organisational boundaries with the health care teams in primary care, secondary care and tertiary care to establish systems of care that support the referral and management of patients that have or require urgent admission with a range of Cardiology conditions

Job responsibilities

The Post holder will work as part of the Cardiology team to provide advanced clinical assessment, support and onward referral for patients attending the Emergency Department, Emergency Assessment Unit, and any department or ward in the OUH sites

The Post holder will support and guide activities; developing and motivating staff education in order to ensure the development of effective, evidence based multidisciplinary practice.

The Post holder is responsible for providing expert practice through leadership, consultation, clinical advice, teaching and support for staff. The post holder will work with clinical colleagues to provide information, advice and support to patients with complex cardiac conditions.

The Post holder will respond to changing needs within the area of cardiology, taking a professional lead in the development of an evidence based approach to practice, fostering a culture that contributes to the clinical governance agenda.

Post Specific Criteria:-

The Post holder will liaise across organisational boundaries with the health care teams in primary care, secondary care and tertiary care to establish systems of care that support the referral and management of patients that have or require urgent admission with a range of Cardiology conditions

The Post holder will provide and facilitate the provision of expert advice and support for patients and their families who have been referred to the OUH NHS Trust.

The post holder will on an ad-hoc and needs-driven basis, provide clinical support to the Cardiology or ACCU teams where needed, to manage acutely unwell patients

The Post holder will provide clinical leadership by supporting the development of professional skills across nursing and other professional groups to ensure that patient pathways meet with national and local requirements

The Post holder will provide clinical support to primary & secondary care colleagues to develop protocols, implement guidance and provide evidence based care for patients referred to the OUH NHS Foundation Trust with a range of cardiology conditions.
