  1. Deliver compassionate excellence via a patient focused clinical pharmacy service, in accordance with national medicines optimisation principles and local clinical pharmacy procedures.
  2. To lead in developing the pharmacy service and adapting to changing service needs on a medical short stay ward.
  3. To lead and support training and development of rotational pharmacy colleagues including trainee pharmacists and the wider multidisciplinary team.
  4. To work closely with the wider pharmacy team to optimise transfer of patient care.
  5. To continually develop your knowledge and be an expert and advocate in acute medicine.
  6. To promote and represent antimicrobial stewardship, medicine effectiveness and medicine safety in Acute General Medicine.
  7. To work collaboratively with the Medicine Pharmacy team in accordance with Trust values.

Main duties of the job

Clinical Practice Role

  • To provide a ward-based clinical pharmacy service according to Trust agreed standards.
  • To maintain a clinical expertise and develop particular areas of interest as appropriate and in accordance with the Trust and Pharmacy strategy.
  • To provide clinical pharmacy cover to other ward areas within the team on a reciprocal basis.
  • To ensure medicines are used appropriately, safely and cost-effectively in accordance with Trust policy, standard operating procedures and medicines legislation.
