Job summary

- Deliver compassionate excellence via a patient focused clinical pharmacy service, in accordance with national medicines optimisation principles and local clinical pharmacy procedures.
- To lead in developing the pharmacy service and adapting to changing service needs on a medical short stay ward.
- To lead and support training and development of rotational pharmacy colleagues including trainee pharmacists and the wider multidisciplinary team.
- To work closely with the wider pharmacy team to optimise transfer of patient care.
- To continually develop your knowledge and be an expert and advocate in acute medicine.
- To promote and represent antimicrobial stewardship, medicine effectiveness and medicine safety in Acute General Medicine.
- To work collaboratively with the Medicine Pharmacy team in accordance with Trust values.
Main duties of the job
Clinical Practice Role
- To provide a ward-based clinical pharmacy service according to Trust agreed standards.
- To maintain a clinical expertise and develop particular areas of interest as appropriate and in accordance with the Trust and Pharmacy strategy.
- To provide clinical pharmacy cover to other ward areas within the team on a reciprocal basis.
- To ensure medicines are used appropriately, safely and cost-effectively in accordance with Trust policy, standard operating procedures and medicines legislation.
