Lead Pharmacist – Clinical Commissioning and High Cost Drugs

Job summary

OVERALL OBJECTIVES:

To deliver on the Clinical Commissioning Pharmacists work plan

Providing required information on Payment by Results (PbR)-excluded and other high-cost drugs (HCD), helping to ensure that the Trust receives appropriate reimbursement

Answering NHSE and CCG queries and challenges regarding PBR excluded (PBRex) charges

To identify and facilitate the implementation of Trust approved QIPP and CQUIN programmes for high cost medicines, and to share schemes with other local providers to ensure consistency across the health economy

To provide professional and independent advice to the Wessex Specialised Commissioning Team pharmacy lead and supplier manager, other commissioners and directorates to ensure that relevant financial and implementation issues for NICE TAGs are identified and articulated at an early stage.

Main duties of the job

  • Lead on the implementation of NHS England commissioning policies
  • Support the Medicines Effectiveness Pharmacist with the implementation of NICETA involving High Cost Drugs
  • To provide the highest care and service delivery to all homecare patients
  • To contribute to and participate in the clinical / ward pharmacy service.

Job responsibilities

Lead on the implementation of NHS England commissioning policies including:

Identification of the financial impact of such recommendations for specialised commissioning

Development of a process to identify, and where appropriate authorise, the providers use of HCMs for individual patients meeting agreed on criteria

Support the Medicines Effectiveness Pharmacist with the implementation of NICETA Ivling HCD.

Development of effective systems for identifying and reviewing impending NICE guidance on High-Cost Drugs (HCDs)

Identify the financial impact of such recommendations with commissioners, including population sizes and costs per capita

Implement and develop a process to identify and where appropriate authorise acute Trust use of high-cost and out-of-tariff medicines, including NICE, for individual patients meeting agreed criteria

Ensure robust policies and procedures are in place to effectively manage Patient Access Schemes (PAS) and related claims/income

Liaise and negotiate with a range of senior colleagues to ensure successful implementation and monitoring of agreed QIPP schemes relating to HCMs.

To propose and lead the implementation of initiatives designed to maximise savings for commissioners (CCG & NHSE).

Work with the provider, Wessex Area Team and service specialists to identify, agree and implement new QIPP proposals involving HCMs.

Work alongside the Medicines Effectiveness and Clinical Effectiveness Pharmacists to ensure horizon scanning is completed and there is a process for Individual Funding Requests (IFR) in the Trust.

Work with the Wessex Area Specialised Commissioning Team, pharmacy lead, the Chief Pharmacist and Medicines Effectiveness Pharmacist to deliver the annual commissioning intentions for HCMs.

Support the development of robust contracting arrangements to promote high quality and cost effective prescribing.

Attend on behalf of the Wessex Area Team appropriate prescribing committees and network groups, such as MMTC, Immunoglobulin Advisory Panel and Area Prescribing Committee of Oxfordshire (APCO).

To ensure operational processes are followed for specific commissioning requirements related to reimbursement of individual medicines.

To provide commissioning support to the specialist pharmacist team leaders, to assist in understanding and implementing commissioning policies and protocols.

To be responsible for the implementation of systems for homecare and overall manage the provision of all Trust homecare services, ensuring that national standards and governance arrangements are in place for all medicines supplied via homecare.

To represent the Trust at the monthly regional homecare procurement meetings

To be the main point of contact between the pharmacy department, Trust staff and Homecare/pharmaceutical companies with regards to medicines supplied via Homecare and other patient access/risk sharing schemes.

Work with senior clinical pharmacists and outpatient staff and other Pharmacy staff to identify, plan, and develop new opportunities for homecare services.

To be the primary point of contact within the Trust between Pharmacists, Clinicians, Purchasers, Contracting and Finance regarding medication supplied via Homecare.

To support the strategic development and planning of drug expenditure reporting

To have a high level of expertise in the use of the Pharmacy databases used to provide usage information on high-cost and other rechargeable drugs.

To resolve specific queries from commissioners on PRBex medicines expenditure information

To support the provision of specialist drug expenditure information and advice to departmental and Trust staff

To support effective communication throughout the pharmacy service and the Trust of any developments in reporting expenditure on PRBex medicines

To implement service changes, policies and procedures for the effective and accurate provision of patient-level drug expenditure information.

To understand the implications and importance of PbR, and of high-cost and other rechargeable drugs - to be able to engage other staff in supporting the provision of information to the Trust Contracts Department and Commissioners.

To support the management of schemes and services (Homecare, drug company patient access schemes) to ensure timely and safe patient access to PRBex medicines.

Build regional links with other pharmacists and technicians in order to share best practice.

To contribute to wider policies that have an impact within the pharmacy department and across the Trust.
