We are currently looking to recruit a Band 6 Patient and Family Liaison Nurse to our Neurosciences Intensive Care Unit.
You will need to be currently working in a critical care unit as either a Band 5 or Band 6. You need to be a dynamic and assertive Critical Care nurse capable of making decisions. You must have critical care experience, a post graduate qualification or relevant experience.
Ideally, you will have a degree, or be committed to obtaining one. You will need to be highly motivated, have good communication skills and experience of leadership within the clinical workplace is essential. In order to work with us, you need to be energetic, self-motivated and have a genuine passion for delivering compassionate nursing and high quality care to the most vulnerable patients and their families.The successful candidate will work closely with their Band 7 team leader to further develop their managerial and leadership skills. Main duties of the job
- To have nursing responsibility to assess and address the psychological needs of families and patients from admission, during their stay and through to discharge from intensive care
- To provide effective nursing leadership, expertise, advice and psychological support to families on intensive care to ensure the provision of high-quality care to patients and families.
- To facilitate referral to appropriate support services within the whole psychological team
- To act as a designated support resource to patients and families and other professionals caring for them
- To provide family bereavement support and follow up care
- To promote an innovative and progressive attitude to the continual improvement of patient and family centred care through research and evidence-based practice
- To work in close co-operation with and communicate effectively at all levels within the multi-disciplinary team (MDT) and outside agencies
- To contribute to long term patient management and discharge planning
- To work 20% clinical as per trust policy
About us
The Neuroscience ICU is a busy Multi-Trauma Critical Care Centre located in the purpose built facility at the Oxford University Hospitals Foundation Trust, based at the John Radcliffe site. It comprises of 15 beds providing specialist services for neurosciences, neurology, trauma and specialist surgery patients. The case mix is varied caring for patients requiring level 2 and level 3 support, with a mix of emergency and complex elective cases.We are a friendly and forward thinking team with a strong commitment to professional development who actively demonstrate the Trust Values. - To provide information, clinical advice and support to patients and families on intensive care
- To be a highly competent, knowledgeable and visible practitioner within intensive care ensuring the provision of high-quality evidence-based nursing care for patients and their families
- Ensure that each patient and family is assessed, and their care is planned, implemented and evaluated in negotiation with the family and other staff caring for the patient, and that this is accurately communicated and documented
- Liaise with clinical, non-clinical and facilities staff in order to maintain a safe, friendly and welcoming environment for the patients, families, visitors and staff
- To act as an advocate for the patients and family, ensuring the provision of appropriate information and support services
- Ensure effective communication between all members of the multi-disciplinary team both internally and externally to the Trust, other hospital departments, relatives and visitors
- Develop and maintain the clinical skills and knowledge in order to provide clinically effective, holistic, evidence-based nursing care in intensive care by working one clinical shift per unit roster. This includes the expanded role and responsibilities of the nurse, within the Scope of Professional Practice.
- To develop and fully utilise clinical assessment and referral skills
- To be competent in the administration of medication under the patient group directives.
- Provide support and counsel to families through hospital at home, with appropriate liaison with other agencies.
- Being involved in discussions where the focus of treatment changes, moving towards palliative care and symptom management
- To support families within the hospital and home environment with appropriate referral and liaison within the multi-disciplinary team
- To work as an expert practitioner to co-ordinate terminal care services using your expertise in palliative care and symptom management
- To offer support to bereaved families and refer on to other services where appropriate
- Establish and maintain internal and external links in order to develop the service
- Maintain national profile through professional forums and contributing to practice developments in a national basis
- To actively contribute to all aspects of service development
- Promote and support the improving working lives philosophy and initiatives
- Provide support to staff in the department
